医院224株肠杆菌科细菌药物敏感性分析

Drug susceptibility analysis of 224 strains of Enterobacteriaceae in the hospital

摘要 目的分析医院224株肠杆菌科细菌对21种抗菌药物的药物敏感情况,指导临床合理用药。方法回顾性分析医院2014年8月-2016年4月临床送检224株肠杆菌科细菌感染的标本,分析肠杆菌科细菌在不同标本以及不同临床科室中的分布情况,采用药敏分析仪进行细菌鉴定及药物敏感性分析,测定肠杆菌科细菌对21种抗菌药物的敏感性。结果 224株肠杆菌科细菌分别来自痰液、尿液、血液、穿刺液、分泌物、脓液和其他,以痰液和尿液检出为主,共158株,占70.54%;哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、亚胺培南的耐药性最低,均<10.00%;氨苄西林耐药率最高,耐药率为84.4%。结论肠杆菌科细菌感染标本多来自呼吸及泌尿系统,对碳青酶烯类、β-内酰胺类抗菌药物β-内酰胺酶抑制剂复方药物耐药率较低,泌尿系统感染的肠杆菌科细菌对喹诺酮类耐药率较高,应引起重视,加强抗菌药物的合理使用。 OBJECTIVE To analyze the drug susceptibility of 224 strains of Enterobacteriaceaeto 21 kinds of antibiotics in the hospital,so as to guide rational use of antibiotics.METHODS A total of 224 strains of Enterobacteriaceaeinfection specimens from Aug.2014 to Apr.2016 in the hospital were retrospectively analyzed.The distribution of Enterobacteriaceaein different specimens and departments were analyzed,the bacteria were identified and their drug susceptibility were analyzed by the susceptibility analyzer and the susceptibility of Enterobacteriaceaeto21 kinds of antibiotics were detected.RESULTS Among 224 strains of Enterobacteriaceae isolates,42.86%(96strains)were isolated from the sputum,27.68%(62strains)from the urine,and 12.05%(27strains)from the blood;The top three departments were internal medicine(68.30%,153strains),surgery(12.50%,28strains)and outpatients(11.16%,25strains),respectively.The drug resistant rates of 224 strains of Enterobacteriaceae to piperacillin/tazobactam,cefoperazone/sulbactam,and imipenem were 1.3%,1.8%,and 4%.respectively,The drug resistant rate to ampicillin was the highest,which was 84.4%.The drug resistant rates to lomefloxacin,levofloxacin,and ciprofloxacin were 47.8%,44.2%,and 46%,respectively.CONCLUSION Enterobacteriaceae are mainly isolated from respiratory system and urinary system,the drug resistant rates were low to carbapenems,β-lactamase/β-lactamase inhibitor,while the drug resistant rates of Enterobacteriaceaeisolated from urine to quinolone were higher than from other specimens.The issue should be paid more attention and we should strengthen the rational use of antibiotics.
