极低出生体重儿喂养耐受性与胃液pH值的观察与分析

Clinical observation and analysis of the pH value of feeding tolerance and intolerance in very low birth weight infants

目的观察和分析极低出生体重儿喂养不耐受的胃液pH值。方法选取2011年1月-2015年9月我院新生儿重症监护室收治住院的86例极低出生体重儿,按诊断标准是否发生喂养不耐受,分为耐受组（Feeding tolerance,FT组）与不耐受组（Feeding intolerance,FI组）,对两组分娩方式、胎龄、出生体质量、性别及母体身体状况、管饲喂养时间、静脉营养天数、住院天数、胃肠营养喂养耐受情况（奶量、喂养容量、胃液pH值、胃残留量）进行比较分析。结果 FT组的胃液pH值为2~5,FI组胃液pH值为3~7。极低出生体重儿胃酸酸度较正常新生儿低（P=0.00）,FI组比FT组胃液pH值高,均数分别是3.05和4.98。结论喂养不耐受组较耐受组胃液pH值水平增高,极低出生体重儿胃酸度较正常新生儿低,不耐受组胃酸度较耐受组低。

Objective To observe and analyze the pH value of gastric juice in very low birth weight infants（VLBW）.Method A totally of 86 hospitalized VLBW infants,who were collected from January 2011 to September 2015 in neonatal intensive care unit in a three grade hospital.According to the diagnostic criteria for feeding intolerance,patients was divided into tolerance group（Feeding tolerance,FT group）and intolerance group（Feeding intolerance,FI group）,The mode of delivery,gestational age,birth weight,gender and maternal body condition,nasal tube feeding time,parenteral nutrition,hospitalization time,enteral feeding tolerance（milk yield,feed volume,pH value of gastric juice and gastric residue）for the two group were compared and analyzed.Results The gastric juice pH value was 2~5in tolerance group,and the pH value of gastric juice was 3~7in intolerance group.Acidity of gastric acid of very low birth weight infant is much lower than that of normal neonates（P=0.00）,The pH value of gastric juice in feeding intolerance group was higher than that of tolerance group.The average number is 3.05 and 4.98 respectively.Conclusion The pH value of gastric juice in feeding intolerance group were much lower that of tolerance group.The pH value of gastric juice in very low birth weight infants was much lower than that of the normal newborn.Gastric acidity in intolerance group was much lower than that of tolerance group.
