支撑喉镜下喉显微手术联合声带内注射治疗声带息肉疗效评价

A curative effect evaluation of the laryngeal microsurgery under laryngoscope combined with vocal cord injection treatment to vocal cord polyps

目的:观察支撑喉镜下喉显微手术联合声带内注射治疗声带息肉的临床疗效。方法:将入选患者以随机数字法分成治疗组及对照组各47例。对照组实施支撑喉镜喉内镜下切除声带息肉手术,治疗组则在息肉或小结摘除后于声带双侧分别注入复方倍他米松注射液2.5mg。观察2组患者手术前后嗓音分析fitter、Shimmer、NNE及HNR参数变化并评价疗效,术后声带粘连、舌麻木、门齿损伤等并发症及术后6个月随访患者复发等情况。结果:治疗组总有效率(93.61%)高于对照组(76.60%),差异有统计学意义(P<0.05)。术后2周治疗组嗓音功能各项参数指标优于对照组(P<0.05);术后6个月随访,治疗组症状复发率(6.38%)低于对照组(19.15%)(P<0.05)。结论:在支撑喉镜显微镜下声带息肉摘除的基础上,加用声带内注射复方倍他米松不仅短期内可以提高临床疗效,改善患者嗓音功能,还可以尽可能减少复发。

[ Objective] Observing the clinical curative effect of the laryngeal microsurgery under laryngoscope combined injection treatment to vocal cord polyps. [ Method]Using the random coding method, 47 patients were randomly divided into treatment and control group . The vocal cord polyps were ablated with laryngeal microsurgery under laryngoscope in the control group, 2.5mg Compound Betamethasone Injection was given in bilateral vocal cord polyps after surgery in the treatment group. Observing the changes of fitter, Shimmer, NNE, HNR and evaluating the complications of vocal cord polyps adhesions, numbness of tongue and fore - tooth impairment after surgery. [Result] Total effective rates were 93.61% and 76.60% for the treatment and control group, respectively. The difference was statistically significant （ P 〈 0.05）. The treatment group voice function parameter index was better than that of control group after two weeks, the difference was statistically significant （ P 〈 0.05）. After 6 months follow- up, the recurrence of symptoms accounted for 19.15% in the control group, 6.38% in the treatment group, the difference was statistically significant （ P 〈 0.05）. [ Conclusion] By the laryngeal microsurgery under laryngoscope combined Compound Betamethasone Injection treatment to vocal cord polyps, it could not only improve the clinical curative effect and the voice function in the short term, but minimize the recurrence as well.
