甲氧氯普胺治疗偏头痛的临床观察

Clinical Observation of Metoclopramide in the Treatment of Migraine

目的:观察甲氧氯普胺治疗偏头痛的疗效和安全性。方法:120例偏头痛患者随机分为观察组(60例)和对照组(60例)。对照组患者给予磁疗、氧疗、心理疏导缓解压力,提示保持健康生活方式,且轻中度患者给予对乙酰氨基酚混悬滴剂2.5 mg,口服,每日1次;重度患者给予2%盐酸利多卡因注射液1 mg/kg复合醋酸地塞米松注射液5 mg,静脉推注(2 min内推完),每日1次。观察组患者在对照组治疗的基础上给予甲氧氯普胺片10 mg,口服,每日1次。两组疗程均为14 d。观察两组患者的临床疗效,治疗前后头痛发作频次评分、头痛发作持续时间评分、头痛发作程度评分、血液流变学指标及不良反应发生情况。结果:观察组患者总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者头痛发作频次评分、头痛发作持续时间评分、头痛发作程度评分、血液流变学指标均显著低于同组治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论:在常规治疗的基础上加用甲氧氯普胺治疗偏头痛的疗效和安全性均较好。

OBJECTIVE： To observe the efficacy and safety of metoclopramide in the treatment of migraine. METHODS： 120 patients with migraine were randomly divided into observation group （60 cases） and control group （60 cases）. Control group received magnetic therapy, oxygen therapy, psychological counseling and maintaining healthy lifestyle and avoiding all kinds of migraine incentives, mild- moderate headache patients were given Paracetamol suspension drop 2.5 rag, orally, once a day, severe patients were given 2% lidocaine hydrochloride injection 1 mg/kg＋dexamethasone acetate injection S mg, intravenously injected within 2 min, once a day. Observation group was additionally given Metoclopramide tablet 10 mg, orally, once a day. The patients were treated for 14 d. Clinical efficacy, headache seizure frequency, headache seizure duration and degree score, hemorbeology indexes before and after treatment, and the incidence of adverse reactions in 2 groups were observed. RESULTS： The total effective rate in observation group was significantly higher than control group, with statistical significance （P〈0.05）. After treatment, the headache seizure frequency, headache seizure duration and degree score, hemorheology indexes in 2 groups were significantly lower than before, and observation group was lower than control group, with statistical significance （P〈0.05）. And there was no significant difference in the incidence of adverse reactions （P〉0.05）. CONCLUSIONS： Based on the conventional treatment, metoclopramide shows good efficacy and safety in the treatment of migraine.
