摘要 目的探讨磷酸三钙(Tricalcium phosphate,TCP)多孔生物陶瓷棒结合自体骨髓间充干细胞(bone marrow stromal cells,BMSCs)移植治疗早期股骨头坏死的近期临床疗效。方法回顾性分析2013年3月至2014年3收治的17例(21髋)股骨头坏死患者,按世界骨循环研究学会(ARCO)标准:ARCOⅠ期6髋;ARCOⅡ期15髋,均采用TCP多孔生物陶瓷棒结合自体骨髓间充质干细胞移植进行治疗。收集患者术前及术后24个月的髋关节Harris评分和影像学资料,并结合二者进行综合评分,以评价术后临床疗效。结果术后24个月所有患者髋关节综合评分为(88.41±5.82)与术前(69.38±4.78)相比差异有统计学意义(P<0.05)。优良率为90.47%术后24个月所有患者髋关节综合评分为(88.41±5.82)与术前(69.38±4.78)相比差异有统计学意义(P<0.05)。优良率为90.47%。结论 TCP多孔生物陶瓷棒结合自体BMSCs移植治疗早期股骨头坏死近期临床疗效肯定。 Objective To investigate the clinical effect of tricalcium phosphate(TCP) porous bio-ceramics rod in combination with autologous bone marrow mesenchymal stem cells (BMSCs) transplantation for the treatment of early stage necrosis of the femoral head. Methods Retrospective analysis of 17 patients (21 hips) with femoral head necrosis, from March 2013 to April 2014, were done. According to association research circulation osseous(ARCO) assessment, I (6 hips) and II term cases (15 hips) were treated with TCP porous bio-ceramics rod combined with autologous BMSCs transplantation. Before and after surgery for 24 months, Harris hip score and radiographic data were collected, and were combined to evaluate postoperative clinical efficacy. Results Hip composite score of all patients after surgery for 24 months showed significant compared to that of all patients before surgery( 88.41 _+ 5.82 vs 69.38 ~ 4. 78 ) (P 〈 0.05 ), and excellent rate reached to 90.47%. Conclusion The short-term efficacy is good that TCP porous bio-ceramic rod in combination with autologous BMSCs transplantation in the treatment of early stage necrosis of the femoral head.
