下尺桡关节非创伤性关节炎伴自发性腕部伸肌腱断裂的诊治

Clinical evaluation and treatment of spontaneous rupture of the extensor tendon associated with non-traumatic distal radioulnar joint osteoarthritis

目的介绍伸指肌腱自发性断裂的临床特点与治疗方法，并对其进行文献综述。方法自2003年3月～2015年2月，对7例伸指肌腱自发性断裂的患者行肌腱转位或自体肌腱移植进行修复，其中5例患者行尺骨头切除术。结果术后随访时间为9-18个月，未见肌腱再次断裂，关节活动度评分（TAM评分）5例优，2例良，优良率为100％。术后肌腱病理组织检查提示肌腱组织非特异性炎症。结论手术修复患者腕部伸指肌腱自发性断裂伴下尺桡关节骨性关节炎可取得良好的临床效果。

Objective To introduce the clinical presentation and treatment of spontaneous rupture of the extensor tendon associated with distal radioulnar joint osteoarthritis and review of the literature. Methods From Mar 2003 to Feb 2015, seven cases presented with spontaneous extensor tendon rupture associated with distal radioulnar joint osteoarthritis were treated. All cases received tendon transfer or tendon palmaris longus autografi. Among the seven patients, five received ulnar head resection. Results All patients were followed-up from 9 to lS months. All patients achieved l00% good out- comes without tendon rupture recurrence. Histopathologically examine showed nonspecific inflammation of the tendon tissue. Conclusion Surgical treatment of spontaneous rupture ofthe extensor tendon associated with distal radioulnar joint osteoarthritis could ~et positive outcomes.
