摘要
目的探讨在经阴道超声评估子宫大小基础上联合应用长椭球体(子宫体)及圆柱体(宫颈)体积公式评估全子宫重量的价值。方法选取81例临床确定需行全子宫切除术患者,术前利用经阴道超声分别测量宫体(长径、左右径、前后径)及宫颈(长径、前后径)各径线,分别按照长椭球体和圆柱体公式计算宫体及宫颈体积,二者相加获得全子宫体积,并根据子宫组织密度计算宫体、宫颈、全子宫重量值。子宫全切术后直视下分别测量实体宫体、宫颈相应径线及重量各参数,并与术前各参数进行比较。结果经阴道超声所测宫体及宫颈各径线及由此推算的子宫宫体、宫颈、全子宫重量值与术后实体测量结果差异无统计学意义(P>0.05)。结论经阴道超声能准确测量宫体及宫颈各径线,长椭球体和圆柱体体积公式可准确估算子宫的实际体积及重量。
Objective To investigate the feasibility of using a formula combining the prolate ellipsoid (uterine corpus) and cylinder (uterine cervix) formulas to estimate the weight of the total uterus based on the the uterine dimensions obtained from the transvaginal ultrasound images. Methods 81 patients were recruited who were to receive complete hysterectomy. The dimensions of the corpus (length,left to right diameter, anteroposterior diameter) and cervix (length, anteroposterior diameter) were preoperatively measured by transvaginal ultrasound, and the total uterine volume was calculated using the prolate ellipsolid formulas for the corpus and the cylinder formulas for the cervix. With those the weight was calculated according to the uterus tissue density. The parameters of the dimensions and weights of the corpus and cervix were messured directly in entity after complete hysterectomy, and compared with preoperative parameters. Results There were no significant statistical differences in both dimensions and weights between ultrasound estimated values and the postoperative measured ones (P 〉 0.05). Conclusion The transvaginal ultrasound can accurately quantify the dimensions of the corpus and cervix, and prolate ellipsoid (corpus) and the cylinder formulas (cervix) can accurately estimate the actual volume and weight of the uterus.
