腹股沟皮瓣移植术在上肢深度烧伤创面整形中的应用效果

Groin Skin Flap Transplantation for Deep Upper Extremity Burn Reconstruction

目的探讨腹股沟皮瓣移植术在上肢深度烧伤创面整形中的应用效果。方法将30例上肢深度烧伤患者按烧伤部位分为2组:对照组15例为手指深度烧伤患者,观察组15例为腕、手部深度烧伤患者。2组均采用早期清创的方法,行腹股沟皮瓣移植术;比较2组治疗后皮瓣断蒂时间、神经功能烧伤缺损评分及临床疗效。结果观察组皮瓣断蒂时间、神经功能烧伤缺损评分均较对照组显著降低[(22.0±2.7)d比(28.8±2.20)d、(4.56±1.47)分比(11.61±1.50)分,均P<0.05],治疗总有效率较对照组显著升高(100.0%比73.3%,P<0.05)。结论腹股沟皮瓣移植术在上肢深度烧伤创面整形中更适用于腕、手部深度烧伤创面的治疗,能尽快消除创面,缩短伤口愈合时间,恢复功能重建,大大降低截肢率。

Objective To investigate the efficacy of groin skin flap transplantation in deep up-per extremity burn reconstruction.Methods Early debridement and groin skin flap transplanta-tion were performed in 15 patients with deep finger burns（control group）and 15 patients with deep wrist andhand burns（observation group）.The pedicle division time,neurological deficit score and clinical efficacy were compared between the two groups.Results Compared with control group,the pedicle division time and neurological deficit score decreased and the total effective rate increased in observation group[（22.0±2.7）days vs （28.8±2.20）days,（4.56±1.47）vs （11.61± 1.50）and 100.0% vs 73.3%,respectively;P 〈0.05].Conclusion Groin skin flap transplantation is more suitable for deep wrist and hand burns than for deep finger burns.It can eliminate wound-surface,shorten woundhealing time,promote functional recovery and reduce amputation rate in patients with deep upper extremity burns.
