温柔分娩管理模式在初产妇分娩中的应用

The Application of Mild Delivery Pattern In Primiparas

目的分析温柔分娩管理模式应用于初产妇分娩中的临床效果。方法选取2015年3月-2016年3月在某院分娩的足月单胎头位顺产且无并发症初产妇200例,其中温柔分娩管理模式接生的初产妇100例为试验组,传统接生法的初产妇100例为对照组。比较两组产妇会阴完整率、侧切率、裂伤程度、会阴部疼痛感、住院日数、盆底肌肉损伤情况等。结果两组患者中均无Ⅲ度及以上会阴裂伤。试验组会阴完整率高于对照组(P<0.05),会阴Ⅰ度撕伤率、会阴侧切率均小于对照组(P<0.05),产时Ⅰ度会阴裂伤明显少于对照组(P<0.05),产妇会阴疼痛感明显轻于对照组(P<0.05),盆底肌肉损伤情况优于对照组(P<0.05),住院日数及住院费用明显少于对照组(P<0.05),差异具有统计学意义。两组比较会阴Ⅱ度撕伤率、Ⅱ度会阴裂伤率差异无统计学意义。结论温柔分娩管理模式产妇创伤小,疼痛感轻,住院日数少,盆底肌肉损伤情况轻,舒适度增加,值得临床推广。

Objective To explore the effect d mild Delivery pattern of gentle delivery to primiparas. Methods Choosing 200 primiparas who had been vaginal delivered from Mar 2013 to Mar 20!4, All pregnant women with single fetus and vertex present had no other medical complications. All of the 100 primiparas had used themild delivery pattern of gentle deliverywere selected as the observing group, and 100 pregnant women had used normal midwifery were selected as the control group. Then to analyze the perineal laceration, pain of perlneal, pelvic floor muscle injury, and so on. Results There was noIII and over perineal laceration intwo groups. The rate of complete perineum in observing group was higher than control group. The I perineal laceration rate and episiotomy rate in observing group were more than control group, was statistically significant （ P〈0. 05 ） . The pain of perineal in observing group was more than control group, was statistically significant （ P〈0. 05 ）. The pelvic floor muscle injury in observing group was a bit better than control group, was statistically significant （ P〈0. 05 ） . The hospitalization time and cost in observing group was less than control group, was statistically significant（ P〈0. 05 ）. Compared with each other about the rate of II perineal laceration, there was no significant difference between them. Conclusion Themild delivery pattern of gentle deliveryto primiparas is little trauma, light aching feeling and＂ pelvic floor muscle injury, short of hospitalization time, improved comfortable degree. It is worth recommend.
