不同糖耐量的良性前列腺增生患者与动脉硬化危险因素的相关性研究 被引量:2

摘要 目的:分析不同糖耐量的良性前列腺增生患者颈动脉内膜中层厚度等指标情况,探讨前列腺体积与动脉硬化危险因素的相关性。方法:选择144例年龄≥50岁的BPH患者,均行口服葡萄糖耐量试验(OGTT),并测量其基线资料、生化指标。应用国际前列腺症状评分(IPSS)评价患者下尿路症状,彩色多普勒超声测量颈动脉内膜中层厚度(IMT)及前列腺大小,并计算前列腺体积(PV)。依据OGTT检测结果分为糖耐量正常(NGT)组、糖尿病(DM)组及糖调节受损(IGR)组;另外根据PV中位数分组,分为前列腺体积较大组和前列腺体积较小组。结果:根据OGTT分组,DM组BMI、IMT水平均高于NGT组、IGR组(P<0.05),IGR组的IMT水平高于NGT组(P≤0.05)。从NGT组到IGR组、DM组,患者PV呈逐渐增高的趋势,且DM组PV高于NGT组(αˊ<0.017)。根据PV中位数分组,前列腺体积较大组患者餐后2h胰岛素(2h Ins)水平、IMT水平明显高于前列腺体积较小组(P≤0.05)。多元线性回归分析结果提示PV与年龄、BMI、2h Ins、IMT呈正相关。结论:动脉硬化危险因素(年龄、BMI、2h Ins、IMT)是前列腺增生发生及进展的高危因素。 Objective: To analyze carotid intima-media thickness( IMT) and related clinical indexes in different state of glucose tolerance of patients with benign prostatic hyperplasia,and assess the relationships between prostate volume( PV) and atherosclerosis risk factors. Methods: The subjects consisted of 144 in patients with BPH,the patient’s age was greater than or equal to 50. All the patients were performed oral glucose tolerance test( OGTT),and to measure the baseline data,biochemical index. Clinical symptoms of patients were evaluated by International Prostate Symptom Score( IPSS). Colour doppler ultrasonography was used to evaluate carotid intima-media thickness( IMT) and the size of the prostate,and calculate the prostate volume( PV).According to oral glucose tolerance test( OGTT) results,the patients were divided into three groups,including the normal glucose tolerance( NGT) group,the impaired glucose regulation( IGR) group,the diabetes mellitus( DM) group. Also according to the median PV level,the patients were divided into the large PV group and the small PV group. Results: In DM group,the BMI and IMT were statistically higher than IGR group and NGT group( P ≤0.05); The IMT were higher in IGR group than NGT group( P ≤0.05). From NGT group to IGR,DM group,PV showed a trend of increased gradually,the PV were higher in DM group than NGT group( α ˊ〈0.017). The 2h Ins and IMT increased significantly in the large PV group compared with the small PV group( P≤0.05). The results showed that PV was positive correlated with age,BMI,2h Ins,IMT by multiple linear regression analysis. Conclusion: Atherosclerosis risk factors( Age,BMI,2h Ins,IMT) are risk factors in the development of prostatic hyperplasia.
