磁共振三维动脉自旋标记灌注成像在不同年龄段正常人脑血流分析中的应用 被引量:7

摘要 目的 探讨磁共振(MR)三维动脉自旋标记(3D-ASL)灌注成像在不同年龄段、不同性别正常人不同脑区脑血流分析中的应用价值。方法 2014年8月-2016年7月间招募39例正常志愿者,行3.0T头颅MR平扫及3D-ASL检查。按年龄分为少年组(11-18岁),青年组(19-39岁)和中年组(40-55岁)三组。ASL图像导入后处理工作站,分析得到全脑血流量(CBF)图,在CBF图上分别在两侧对称位置额、顶、颞、枕叶皮层及白质,小脑半球及脑干选取感兴趣区(ROI),记录各ROI的CBF值。比较脑组织两侧对称位置ROI的CBF值,比较男、女不同性别间各ROI的CBF值,并比较不同年龄三组间各ROI的CBF值。结果 两侧额、顶、颞、枕叶皮层及白质,小脑半球的CBF均无统计学差异。女性各脑区CBF值均稍高于男性,然而两组间各对应脑区的CBF均无统计学差异。少年组、青年组两组额、顶、颞、枕叶皮层及小脑半球CBF值明显高于中年组(P均〈0.05),青年组仅额叶皮层CBF值明显低于少年组(P〈0.05)。结论 3D-ASL可定量测量各脑区CBF值,临床运用价值高。正常人两侧CBF对称,无性别差异,脑皮层CBF随着年龄增长值有随之下降趋势,额叶皮层最早显现。 Objective To explore the application value of magnetic resonance (MR) 3 D arterial spin labeling (3 D-ASL) perfusion imaging in analysis of different brain regions of cerebral blood flow from different ages, different gender normal people. Methods 35 normal volunteers were recruited to do 3.0 T head MR scan and 3D-ASL examination from August 2014 to July 2016 and were divided to juvenile group (11 -18 years old), young group (19 - 39 years old) and middle-aged group (40 -55 years old) based on different age. We imported ASL images to post-processing Workstation to get the total cerebral blood flow (CBF) images, and selected regions of interesting (ROI) at bilateral symmetrical frontal, parietal, temporal and occipital lobe of cortex and white matter, cerebellum and brain stem on the CBF map, recorded the CBF value of each ROI, and compared CBF value of bilateral cerebral symmetry position ROI, each ROI between male and female and each ROI among three different age groups. Results Bilateral CBF values of the frontal, parietal, temporal and occipital lobe of cortex and white matter, cerebellar hemisphere had no statistical difference. The CBF values of female in various brain regions were slightly higher than male, however, there was no statistical difference between male and female groups. The CBF values of the frontal, parietal, temporal and occipital lobe of cortex, cerebellum hemisphere of juvenile group and young group were significantly higher than that of middle-aged group ( all P 〈 0.05 ). In young group, only the CBF value of the frontal cortex was significantly lower than juvenile group ( P 〈 0.05 ). Conclusion 3D-ASL can quantitatively measure the CBF values of various brain regions with a high clinical application value. In normal people, the bilateral CBF are symmetrical and have no gender differences, the CBF value of cerebral cortex has a decreasing trend with increased age and the frontal cortex appears at the earliest.
