Analysis of small shadow distribution in silicosis patients by CT imaging

摘要 目的通过量化数据分析研究矽肺小阴影分布在CT影像中的纵向和横向分布特征以及两肺分布特征。方法回顾性分析30例矽肺患者左右两肺的CT影像学资料,从肺部纵向分区和横向分区2个维度分别计算CT图像上矽肺小阴影分布的面积与相应肺区总面积的比值;然后分别对这个维度的比值数据组进行统计学数据分析。结果 30例矽肺患者小阴影分布的面积与相应肺区总面积的比值,纵向分布:上肺野(24.33%)、中肺野(21.30%)均大于下肺野(11.89%),组间两两比较上-下肺野、中-下肺野差异有统计学意义(P<0.01);横向分布:中带(25.96%),内带(13.13%),外带(18.69%),中带大于内带、外带,组间两两比较中-内带、中-外带差异有统计学意义(P<0.01);右左肺比较:右肺(19.75%)均值稍大于左肺(18.59%),差异无统计学意义(P>0.05)。结论矽肺小阴影在CT影像中的差异特征:上肺野、中肺野大于下肺野,中带大于内带、外带,右肺与左肺无差异,能够为矽肺诊断提供重要依据。 [Objective]To investigate the vertical and transverse distribution,and distribution characteristics in pulmones of silicosis small shadow in CT imaging through the quantitative data analysis. [Methods]CT imaging data of left?and?right?lungs of30 silicosis patients were analyzed retrospectively. The ratios of silicosis small shadow area to total area of lung in CT imaging were calculated from vertical and transverse dimensions respectively. And then the results were analyzed statistically.[Results]For the vertical distribution,the ratio of upper field(24.33%)and middle field(21.30%)were higher than that of lower field(11.89%),and the differences were statistically significant between upper field and lower field,middle field and lower field(P〈0.01). For the transverse distribution,the ratio of middle zone(25.96%)was higher than that of inside zone(13.13%)and outer zone(18.69%),and the differences were statistically significant between middle zone and inside zone,middle zone and outer zone(P〈0.01). For the left and right lung,the value of right lung(19.75%) was slightly higher than that of left lung,and the difference was not statistically significant(P〈0.05). [Conclusion]The distribution characteristics of silicosis small shadow in CT imaging are as follows: the ratios of upper field and middle field are higher than that of lower field,the ratio of middle zone is higher than that of inside zone and outer zone,and there is no difference between left and right lung. The results provide important basis for the diagnosis of silicosis.
