摘要
传统抗肿瘤血管疗法是通过破坏肿瘤血管阻断其营养供应而达到抑制肿瘤生长的目的,然而长期抗血管药物的运用会使肿瘤血管出现严重的退化而阻碍药物和氧的传输,导致肿瘤细胞对化疗的耐药及对放疗的敏感性降低。"血管正常化"理论则认为通过合理使用抗血管生成药物,能使肿瘤血管在消退之前出现短暂的趋于正常的时间窗,在此时间窗内联合放化疗治疗,可以更有效地发挥其抗肿瘤的效应。全文就血管正常化理论的提出,血管正常化的机制,尤其对血管正常化理论在临床消化系统肿瘤的运用及面临的问题展开综述。
The traditional anti-angiogenic therapy is to block the nutrient and oxygen supply by inhibiting new blood vessels generation in cancer tissue. However,the long-term use of anti-angiogenic agents would induce resistance in tumor tissue and reduce sensitivity to radiation treatment when the severe regression of tumor blood vessels occurs. The theory of "normalization of tumor vascular" shows that antiangiogenic therapy may transiently "normalize" the tumor vasculature to enhance the inhibitory effects of chemoradiotherapy during the normalization time window by the reasonable use of antiangiogenesis drugs. In this article,we will discuss the theory and mechanism of "normalization of tumor vascular",as well as the application of this theory in clinical management of digestive tract tumor,and the problems it may confront.
