摘要
目的探讨维生素D(VD)水平对哮喘患儿T细胞水平及免疫球蛋白水平影响。方法选取2014年-2015年在绍兴市中心医院就诊及住院的2岁~6岁支气管哮喘患儿60例,随机分为实验研究组(哮喘1组)及实验对照组(哮喘2组),各30例。哮喘1组在常规治疗的基础上给予维生素D3补充3个月。并选取门诊体检健康儿童60例作为健康对照组。对哮喘患儿治疗前后及健康组儿童体检时用化学发光法测定VD水平,免疫比浊法测定Ig A、Ig M和Ig G,流式细胞仪测定CD4+、CD8+、CD4+/CD8+T淋巴细胞水平。结果哮喘患儿VD水平及CD4+水平、CD4+/CD8+及Ig A水平均低于对照组,差异有统计学意义(P〈0.05)。哮喘1组及哮喘2组治疗后CD4+/CD8+、Ig A高于治疗前,治疗后哮喘1组VD水平、CD4+、CD4+/CD8+、Ig A均高于哮喘2组,差异均有统计学意义(P〈0.05)。VD水平与CD4+/CD8+、Ig A均呈正相关(r值分别为3.86、3.93,P〈0.05)。结论哮喘患儿普遍存在维生素D缺乏、T细胞水平及免疫球蛋白水平异常,通过补充维生素D可以改变患儿体内T细胞水平及免疫球蛋白水平。
Objective To discuss the influence of vitamin D( VD) levels on T cell levels and immunoglobulin levels in children with asthma. Methods 60 children aged 2-6 years with bronchial asthma for clinical and hospitalization treatment in Shaoxing Cental Hospital from 2014 to 2015 were selected and randomly divided into the experimental group( group 1) and the control group( group 2),with 30 cases in each group respectively. The experimental group was given vitamin D3 supplement for 3months on the basis of conventional treatment. 60 healthy children were selected as the healthy control group. The vitamin D levels of the children with asthma before and after treatment and healthy children with physical examination were detected by chemiluminescence method,Ig A,Ig M and Ig G were detected by immune urbidimetry,and CD4+,CD8+and CD4+/CD8+were detected by flow cytometry instrument. Results The levels of CD4+,CD4+/CD8+and Ig A of children with asthma were significantly lower than that of the healthy controls,and the differences were statistically significant( P 〈 0. 05). The levels of CD4+/CD8+and Ig A after treatment were higher than that before treatment in asthma group 1 and 2,and after treatment the levels of VD,CD4+,CD4+/CD8+and Ig A in asthma group 1 were higher than that in the asthma group 2,and the differences were statistically significant( P 〈 0. 05). VD levels were positively correlative with CD4+/CD8+and Ig A( r = 3. 86,3. 93,P 〈0. 05). Conclusion Vitamin D deficiency and abnormalities of T cell levels and immunoglobulin were common in children with asthma. Vitamin D supplementation can change T cells levels and immunoglobulin levels in children with asthma.
