摘要
目的分析吉林省耐多药结核分枝杆菌利福平耐药相关基因rpoB的突变特征,为耐多药结核病快速诊断方法的建立提供科学依据。方法采用DNA测序技术对吉林省287株耐多药结核分枝杆菌进行rpoB基因序列检测,分析检测菌株rpoB基因突变特征。结果 287株耐多药结核分枝杆菌中201株耐多药结核分枝杆菌rpoB基因发生突变,突变率为70.03%(201/287)。碱基突变发生在前3顺位的是rpoB 531(39.72%)、rpoB 526(18.82%)、rpoB 516(5.92%)。287株耐多药结核分枝杆菌rpoB基因共发现碱基突变、缺失、插入等40种基因突变类型。其中,单位点碱基突变、双位点碱基突变、多位点碱基突变、碱基缺失、碱基插入的发生率分别为60.63%(174/287)、6.27%(18/287)、1.39%(4/287)、1.39%(4/287)、0.70%(2/287)。结论吉林省耐多药结核分枝杆菌利福平耐药相关基因最常见突变为rpoB 531、rpoB 526和rpoB 516。
Objective To analyze the rifampicin-resistant rpoB gene mutation characteristics of multidrug-resistant Mycobacterium tuberculosis( MDR-TB),so as to provide scientific basis for the establishment of rapid diagnostic methods in Jilin.Methods Using DNA sequencing technology to test rpoB gene sequences of 287 strains of multidrug-resistant Mycobacterium tuberculosis,and to test and analyze the characteristics of rpoB gene mutations. Results Among 287 MDR-TB strains,rpoB gene mutation occurred in 201 strains,with the mutation rate of 70. 03%( 201/287). The top 3 mutations were at rpoB 531( 39. 72%),rpoB 526( 18. 82%) and rpoB 516( 5. 92%). 40 types of mutations,including base mutation,missing,insert in287 strains of MDR-TB rpoB gene. Among them,the incident rates of base unit point mutation,double site base mutation,bases the incidence of base mutations,loss of the bases,the bases insert were 60. 63%( 174/287),6. 27%( 18/287),1. 39%( 4/287),1. 39%( 4/287),0. 70%( 2/287). Conclusion The most common rpoB gene mutations associated with rifampicin resistance in MDR-TB strains in Jilin were at rpoB 531,rpoB 526,rpoB 516.
