摘要
目的探讨Bobath疗法联合引导式教育对不同类型脑瘫病儿生活自理能力的影响。方法选择2012年6月—2014年6月我院收治的脑瘫病儿160例作为研究对象,随机分为观察组100例和对照组60例,观察组采用Bobath疗法联合引导式教育进行治疗,对照组采用单纯Bobath疗法进行治疗,比较两组的治疗效果。结果治疗后观察组独立性评定量表(WeeFIM)评分高于对照组,差异有显著性(t=8.04,P〈0.01);两组治疗前后WeeFIM评分差值比较,差异有显著性(t=10.29,P〈0.01)。治疗后观察组日常生活能力评定量表(ADL)评分高于对照组,差异有显著性(t=13.06,P〈0.01);治疗前后两组ADL评分差值比较,差异有显著性(t=13.83,P〈0.01)。痉挛型病儿治疗前、后WeeFIM与ADL评分及治疗前后差值均显著高于其他类型病儿(F=4.27~7.17,P〈0.01)。结论 Bobath疗法联合引导式教育能够有效改善脑瘫病儿的生活自理能力以及独立性,尤其对痉挛型脑瘫病儿的疗效更为显著,值得临床推广应用。
Objective To explore the impact of Bobath therapy combined with guided education on self-care ability in children with different types of cerebral palsy. Methods A total of 160 children with cerebral palsy treated in our hospital,during June 2012—June 2014,were selected as research objects.The sick kids were divided into observation group(n=100)and control group(n=60)in random.The sick kids in the observation group were treated with combined Bobath therapy and guided education,those in the control group treated with simple Bobath therapy.The effectiveness was compared between the two groups.Results After treatment,the score of independent rating scale(WeeFIM)in the observation group was higher than that in the control group(t=8.04,P〈0.01).The difference in WeeFIM score between before and after treatment was significant(t=10.29,P〈0.01).After treatment,the ADL score in the observation group was higher than that in the control group(t=13.06,P〈0.01).A difference comparison in the two groups between before and after treatment showed the difference was significant(t=13.83,P〈0.01).WeeFIM and ADL score,and defference comparison in children with spastic type,before and after treatment,were all higher than that in other types of sick children(F=4.27-7.17,P〈0.01). Conclusion Bobath therapy combined with guided education can effectively improve self-care ability and independence in children with cerebral palsy,especially for spastic cerebral palsy.This combination therapy is worthy of clinical popularization and application.
