个体化护理对小肝癌射频消融术患者治疗的影响

INFLUENCE OF INDIVIDUALIZED NURSING ON PATIENTS WITH SMALL HEPATOCELLULAR CARCINOMA BY RADIO FREQUENCY ABLATION

目的探讨个体化护理对小肝癌射频消融术(radio-frequency ablation,RFA)患者治疗效果的影响。方法选取2014年10月—2016年10月唐山市人民医院收治的58例诊断为小肝癌且实施射频消融术的患者,按照患者及家属知情同意和自愿原则分为对照组26例、观察组32例。对照组实施常规手术护理,观察组在对照组基础上实施个体化护理干预。比较分析二组患者心理状态、并发症及疼痛的改善效果。结果个体化护理干预后,观察组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分(24.87±2.74,25.44±3.12)均低于对照组(38.14±3.22、44.56±3.84),观察组术后发热、腹腔及胆道并发症发生率(9.4%、3.1%)明显低于对照组(34.6%、23.1%),观察组疼痛视觉评分(VAS)(28.55±3.79)评分明显比对照组(52.48±4.46)低,差异均有统计学意义(P<0.05)。结论对小肝癌射频消融术患者实施个体化护理干预,具有改善患者心理状态,减少术后发热、腹腔及胆道并发症的发生,减轻患者疼痛等效果。

Objective To explore the effect of individualized nursing on patients with small hepatocellular carcinoma undergoing radio frequency ablation. Methods The diagnosis of 58 cases were selected from Oc- tober 2014 to October 2016 in author＇s hospital for small hepatocellular carcinoma and radiofrequency abla-tion in patients with informed consent of patients and their families and voluntarily divided into 26 cases in the control group, 32 cases in the experimental group. While patients of experiment group were treated with individualized nursing except for regular nursing. The psychological state, complications and pains of the two groups of patients were compared and analyzed. Results After given individualized nursing, scores of SAS and SDS of experimental group were respectively （24.87±2.74） points and （25.44±3.12） points, lower than those of the control group, which were （38.14±3.22） points and （44. 56±3.84） points. The incidence of postoperative fever, abdominal cavity and biliary complications in experiment group was significantly lower than that of control group [9.4% vs 34.6%, 3.1% vs 23.1%）. Scores of VAS in experiment group was significantly lower than that of control group （28.55±3.79 vs 52.48±4. 46）, the comparative differences were all statistically significant （P〈0.05）. Conclusions The implementation of individualized nursing intervention for patients with small hepatocellular carcinoma radio frequency ablation can effectively improve the psychological status of patients, reduce the abdominal cavity and biliary complications of surgery and reduce the pain of patients and so on.
