摘要
目的研究缺血性脑血管病患者血管内成型支架治疗及内科抗凝治疗效果。方法选择2013年1月至2015年1月期间,以第三军医大学附属新桥医院神经内科的80例缺血性脑血管病患者作为研究对象,采用随机数表法分为两组,每组40例。观察组行血管内成形支架手术治疗,对照组单纯行内科抗凝治疗。观察两组患者治疗后神经功能缺损程度、血管狭窄严重程度及不良反应事件发生率。结果观察组治疗结束时神经功能缺损(NIHSS)评分为(12.43±2.08)分,显著低于治疗前和对照组治疗后(P<0.05)。观察组治疗后血管狭窄<60%患者显著多于对照组(P<0.05)。观察组治疗后患者死亡率和不良反应发生率分别为2.5%和12.5%,显著低于对照组的15%和47.5%,(P<0.05)。结论血管内支架成形术较内科常规抗凝治疗缺血性脑血管病疗效更为显著,能显著改善血管狭窄和神经功能低下状态,且手术不良反应少,安全性高。
Objective To study treatment effects of endovascular stent surgery and anticoagulation in internal medicine in the treatment of patients with ischemic cerebrovascular disease. Methods 80 patients with ischemic cerebrovascular disease from January 2013 to January 2015 in Xinqiao Hospital were selected as the study subjects,they were randomly divided into two groups( each 40 cases). The observation group underwent endovascular stent surgery,the control group underwent simple anticoagulation in internal medicine. Degree of nervous functional defects,severity of angiostegnosis and incidence of adverse events in the two groups after the treatment were observed. Results Neurologic impairment( NIHSS) score was( 12. 43± 2. 08) scores in the observation group till the end of the treatment,which was significantly lower than that before the treatment and the control group after the treatment( P〈0. 05). Angiostegnosis in the observation group after the treatment was less than 60%,which was significantly more than the control group( P〈0. 05).Mortality and incidence of adverse reactions in the observation group after the treatment were 2. 5% and12. 5% respectively,which were significantly lower than the control group[( 15%),( 47. 5%) ]( P〈0. 05).Conclusion Efficacy of endovascular stent surgery is more significant compared with conventional anticoagulation in internal medicine in the treatment of ischemic cerebrovascular disease,can improve angiostegnosis and low nerve function significantly,less operation adverse reactions and high safety.
