荧光原位杂交技术在上尿路和下尿路尿路上皮癌诊断应用中的对比 被引量:4

摘要 目的比较荧光原位杂交技术(FISH)在上尿路和下尿路尿路上皮癌中的诊断效能。方法疑为尿路上皮癌的患者共155例(包括上尿路占位56例,其中尿路上皮癌49例;下尿路占位99例,其中尿路上皮癌75例),进行FISH检测,进行敏感性、特异性等方面的比较。结果FISH在上尿路和下尿路尿路上皮癌中的敏感性分别为87.8%和72.0%(P〈0.05),特异性分别为85.7%和70.8%(P〉0.05),阳性预测值分别为97.7%和88.5%(P〉0.05),阴性预测值分别为50.0%和44.7%(P〉0.05)。结论FISH在上尿路尿路上皮癌中的敏感性和阳性预测值高于下尿路尿路上皮癌,而特异性和阴性预测值相仿,与上尿路癌恶性程度较高,诊断时分期较晚有关。 Objective To compare the efficacy of the application of fluorescence in situ hybridiza- tion (FISH) for the diagnosis of urothelial carcinoma (UC) of upper urinary tract versus lower urinary tract. Methods 155 patients suspected UC were divided into two groups: upper urinary tract group (56 cases, 49 of them confirmed with upper urinary tract UC) and lower urinary tract group (99 cases, 75 of them confirmed with UC of lower urinary tract). Both groups underwent FISH test. The sensitivity, speci- ficity, positive predictive value and negative predictive value were compared between two groups. Results Sensitivities of FISH in diagnosing UC of upper and lower urinary tract were 87. 8% and 72. 0% (P 〈 0. 05), and specificities were 85.7% and 70. 8% (P 〉 0.05 ), positive predictive values of FISH test were 97. 7% and 88. 5% ( P 〉 0. 05 ), and negative predictive values were 50. 0% and 44. 7% ( P 〉 0. 05), respectively. Conclusion FISH has higher sensitivity and positive predictive value in detecting upper urinary tract UC than Dower urinary tract UC, which is associated with more advanced stage and high- er grade of upper urinary tract UC in this study group.
