诱导痰Runt相关基因3基因CpG岛甲基化在肺腺癌病情及预后评估中的价值

Value of runt related transcription factor 3 gene CpG island methylation in induced sputum in con- dition and prognosis evaluation for lung adenocarcinoma

目的探讨诱导痰Runt相关基因（Runx）3基因CpG岛甲基化在肺腺癌病情及预后评估中的价值。方法选择肺腺癌患者（LAC组）和非肿瘤患者（CON组）为研究对象，采用甲基化特异性聚合酶链反应（MSP）检测LAC组和CON组患者诱导痰Runx3基因CpG岛甲基化并比较其差异，比较LAC组肺腺癌患者不同临床病理特征中Runx3基因CpG岛甲基化的差异。比较LAC组患者诱导痰Runx3基因CpG岛甲基化与未甲基化患者R0切除率、无病生存期和3年生存率的差异。结果LAC组肺腺癌患者诱导痰Runx3基因CpG岛甲基化率显著高于CON组非肿瘤患者（62．5％比2．5％，P〈0．05）。LAC组肺腺癌患者在胸腔积液、分化程度、肿瘤最大径、淋巴结转移、远处转移和cTNM分期中的差异有统计学意义（χ2分别为5．861、4．417、5．273、4．110、10．240、10．559，均P〈0．05）。LAC组肺腺癌患者中Runx3基因CpG岛甲基化者R0切除率（72．0％比93．3％）、无病生存期[（1．7±0．4）年比（2．4±0．6）年]和3年生存率（40．0％比66．7％）均显著低于Runx3基因CpG岛未甲基化患者（χ2=4．083、t=6．261;χ=4．320，P〈0．05）。结论肺腺癌患者诱导痰Runx3基因CpG岛甲基化与病情及预后相关。

Objective To study the value of runt related transcription factor 3 （Runx3） gene CpG island methylation in induced sputum in condition and prognosis evaluation for lung adenocarcinoma. Meth- ods Methylation specific polymerease chain reaction （MSP） was used to detect the Runx3 gene promoter methylation by induced sputum in patient with lung adenocarcinoma （ LAC group） and non - cancer pa- tients （CON group）. The Runx3 gene CpG island methylation rate in different clinicopathologieal factors was compared. The R0 radical resection, disease - free survival time and 3 - year survival rate between pa- tient with Runx3 gene CpG island methylation and unmethylation were compared. Results The Runx3 gene promoter methylation rate in LAC group was significantly higher than in CON group （62. 5% vs. 2. 5% , P 〈 0.05 ）. The Runx3 gene promoter methylation rate in pleural effusion, degree of differentiation, tumor diameter, lymph node metastasis, distant metastasis and cTNM staging were significantly different （χ2 = 5.861, 4.417, 5.273, 4. 110, 10.240, 10.559, all P〈0.05）. The R0 radical resection （72.0% vs. 93.3 % ）, disease - free survival time [ （ 1.7 ± 0. 4） years vs. （ 2.4 ± 0. 6） years ] and 3 - year survival rate （40. 0% vs. 66. 7% ） in patient with Runx3 gene CpG island methylation were significantly lower than in patients with Runx3 gene CpG island unmethylation （χ2 =4. 083 ,t =6. 261 ,χ2 =4. 320, all P 〈0.05）. Conclusion Runx3 gene CpG island methylation in induced sputum correlates with lung adenocareinoma＇ s condition and prognosis.
