乳腺癌前哨淋巴结活检的临床应用价值分析

目的探究乳腺癌前哨淋巴结活检的临床应用价值。方法选取乳腺癌患者68例,时间为2013年1月~2014年1月,分为两组,其中对照组乳腺癌患者直接接受乳腺癌改良根治术治疗,实验组乳腺癌患者则进行全乳腺切除＋前哨淋巴结活检,前哨淋巴结阳性者进一步行腋窝淋巴结清扫,前哨淋巴结活检阴性者免于腋窝淋巴结节清扫。对两组乳腺癌患者术后并发症发情况进行对比。结果实验组乳腺癌患者的肩关节功能评分、两侧上臂臂围差、肩关节外展角度变化情况较对照组优（P〈0.05）。两组乳腺癌患者在术后均出现肿胀、麻木、感觉减退等并发症情况,但是实验组并发症的发生率较对照组低（P〈0.05）,术后随访2年,两组腋窝淋巴结复发率差异无统计学意义（P〉0.05）。结论乳腺癌前哨淋巴结活检应用价值较高,可以使部分乳腺患者免于腋窝淋巴结清扫,降低乳腺癌患者术后并发症的发生率,促进乳腺癌患者的健康恢复,改善乳腺癌患者术后生存质量。

Objective To explore the clinical value of sentinel lymph node biopsy about breast cancer. Methods To select 68 cases of breast cancer from January 2013 to January 2014,divided patients into two groups. The control group patients with breast cancer used breast cancer modified radical treatment directly. Experimental group patients with breast cancer used the whole breast removal and sentinel lymph node biopsy,the sentinel lymph node biopsies positive patients was further used axillary lymph node cleaning,sentinel lymph node biopsy negative patients without axillary lymph nodes cleaning. Compared the postoperative complications between two groups＇ patients. Results The shoulder joint function score,both sides of the upper arm circumference and Shoulder abduction Angle change of experimental group were better than the control group（P〈0.05）. Two groups all had swelling,numbness,hypoesthesia and other complications after surgery. But the chances of complications in the experimental group were lower than control group（P〈0.05）. Postoperative follow-up of two years,There was no significant difference in the recurrence rate between the two groups of axillary lymph nodes（P〉0.05）. Conclusion Breast cancer sentinel lymph node biopsy have higher application value,can make some relatively early breast patients escape from axillary lymph node cleaning,reduce the risk of breast cancer patients postoperative complications,promote the health of the patients about breast cancer recovery,and improve postoperative quality of life about breast cancer patients
