延续护理对脑卒中患者家庭主要照顾者照顾压力与能力影响的研究

Effect of Transitional Care on the Stress and the Competence of the Primary Family Caregivers of the Stroke Victims

目的 探讨延续护理模式对脑卒中患者家庭主要照顾者照顾压力和照顾能力的影响。方法 选取2014年4月~2015年4月在我科住院的脑卒中患者的家庭主要照顾者进行研究,随机分为观察组和对照组。出院后对照组25例采用常规护理措施,观察组26例在常规护理的基础上采用延续护理措施,比较两组患者家庭主要照顾者的照顾压力与能力状况。结果 观察组照顾者的身体、社交状况和心理状况的得分低于对照组,差异有统计学意义（P〈0.05）;与对照组相比,观察组照顾者对脑卒中患者实施下列活动：活动肢体、关节,饮食、营养,按摩、推拿,预防肺部感染,避免脑卒中发生的危险因素,发现脑卒中再发的征兆的正确护理率均高于对照组,差异有统计学意义（P〈0.05）。结论 延续护理模式能降低脑卒中患者家庭主要照顾者的照顾压力,并提高其照顾能力,对改善脑卒中患者的预后和促进其康复有着重要意义,落实了人文关怀理念。

Objective To evaluate the effect of transitional care on the stress and the competence of the primary family caregivers of the stroke victims. Methods From April 2014 to April 2015, primary caregivers who would take care for the patients with stroke treated in our department were recruited. There were divided into experimental group and control group according to the odevity of the last number of their ID ntnnber. Then the routine discharge guidance was given to the controlled group （ 25 participants ）, while the participants in experinaental group （ 26 participauts ） received additional transitional care. The stress of＂ the caregivers was measured by the Chinese version of＂ caregiver stress index （ CSI ） through self-evaluation, and the competence of＇the caregivers was assessed by the questionnaire of＂ caring quality through the nurses. Results Compared to the control group, the scores of the physical, psychological and social status of CSI among the primary fanfily caregivers in the experimental group were lower （P 〈 0.05 ） . There were more numbers of caregivers in experimental group has known how to help stroke victims take exercise, reinforce nutrition, prevent pulmonary int＇ection, avoid risk factors for stroke occurred, and found signs of stroke recurrence （ P 〈 0.05 ） . Conclusion Transitional care could reduce the stress of the caregivers and improve their competence in the caring of the stroke victims, which is important for the prognosis and rehabilitation of the stroke victims.
