33945例宫颈病变的筛查结果分析及DNA倍体定量分析技术的应用价值 被引量:2

摘要 目的 分析33 945例宫颈病变的筛查结果,并探讨DNA倍体定量分析技术在宫颈病变筛查中的应用价值。方法 纳入2012年至2015年该院行宫颈病变筛查的妇女33 945例,年龄18~74岁,取宫颈细胞标本,同时进行常规细胞学检查诊断及DNA倍体定量分析,筛查结果阳性者再进一步行阴道镜下宫颈活检组织病理学诊断。结果 2012~2015年宫颈病变筛查阳性率呈降低趋势(P〈0.05)。DNA倍体分析系统筛查宫颈病变的阳性率为9.28%,高于常规细胞学的6.82%(P〈0.05)。DNA倍体分析的活检阳性率为30.52%,高于常规细胞学的18.22%(P〈0.05);DNA定量分析的宫颈癌阳性检出率为0.144%,也高于常规细胞学的0.076%(P〈0.05)。结论 该院就诊妇女宫颈病变检出率有逐年下降趋势。DNA定量分析法可提高宫颈癌及癌前病变的早期筛查率及活检阳性率。 Objective To analyze the result of 33 945 cases of cervical diseases screening,and to explore the application value of DNA quantitative cytology. Methods A total of 33 945 women, aged 18 to 74 years, receiving cervical diseases screening from 2012 to 2015 were enrolled. The specimens of cervical cells were collected and assayed using and routine cytology examination and DNA quantitative cytology analysis. The patients with positive screening result received cervical biopsy under colposcope for histopathological diagnosis. Results The positive screening rate for cervical diseases from 2012 to 2015 tended to decrease(P 〈0.05). The positive rate of DNA quantitative cytology for cervical diseases screening was 9.28%, higher than that of routine cytology ( 6. 82%, P 〈 0. 05 ). The biopsy positive rate of DNA quantitative cytology was 30. 52%, higher than that of routine cytology ( 18.22%, P 〈 0.05). The positive detection rate of DNA quantitative cytology for cervical cancer was 0. 144% , higher than that of routine cytology ( 0. 076% , P 〈 0.05 ). Conclusion The detection rate of cervical disease shows a decreasing tendency year by year in this hospital. DNA quantitative cytology can improve early screening positive rate and biopsy positive rate of patients of both cervix cancer and precancerous lesions.
