摘要
目的 探讨ω-3多不饱和脂肪酸对结肠癌根治术后化疗患者营养及生活质量的影响。方法 收集2014年6月-2016年1月在我院行结肠癌根治术需要化疗患者。所有患者采用国产奥沙利铂(L-OHL)联合亚叶酸钙(CF)及氟脲嘧啶(5-Fu)组成的m FOLFOX6方案化疗,每14天重复1次,连用12个周期。实验组:化疗期间予中/长链脂肪乳1.0 g/(kg·d)的基础上加用10%鱼油脂肪乳剂100 m L营养支持×2 d。对照组:化疗期间单纯给予中/长链脂肪乳剂1.2 g/(kg·d)营养支持×2 d。比较两组的临床疗效,以及两组患者治疗前后TP、ALB、TF水平、生活质量评分的变化情况。结果 观察组的总有效率为53.3%,显著高于对照组的33.3%,差异有统计学意义(P〈0.05),治疗后观察组患者的TP、ALB、TF水平均较对照组改变更显著。观察组患者治疗后生活质量的躯体功能、角色功能、情绪功能、认知功能、社会功能评分分别显著高于对照组(P〈0.05)。观察组患者治疗后生活质量的疲倦、恶心呕吐、疼痛评分分别显著低于对照组(P〈0.05)。结论 ω-3多不饱和脂肪酸用于结肠癌根治术后化疗疗效确切,能明显改善患者的营养水平及生活质量,有较高的临床推广价值和意义。
Objective To investigate the effects of ω-3 polyunsaturated fatty acids on nutrition and quality of life of patients undergoing chemotherapy after radical surgery of colon cancer. Methods The patients in need of chemotherapy who completed radical surgery of colon cancer from June 2014 to January 2016 in our hospital were collected. All pa- tients were treated with mFOLFOX6 regimen consisting of domestic oxaliplatin(L-OHL) combined with folinic acid(CF) and fluorouracil(5-Fu). The treatment was repeated every 14 days for 12 cycles. Experiment group: Long-chain fat emulsion 1.0 g/(kg·d) was supplemented with 10% fish oil fat emulsion with 100 mL nutrition support for 2 days during the chemotherapy. Control group: 1.2 g/(kg·d) nutritional support of medium/long chain fat emulsion was given for 2 days during the chemotherapy. The clinical effects of the two groups were compared, and the changes of TP, ALB, TF and quality of life scores before and after treatment were compared. Results The total effective rate in the observation group was 53.3%, which was significantly higher than that of 33.3% in the control group (33.3%), and the difference was sta- tistically significant(P〈0.05). The levels of TP, ALB and TF in the observation group were significantly changed than those in the control group (P〈0.05).The physical function, role function, emotional function, cognitive function and social function score of the observation group were significantly higher than those in the control group (P〈0.05). The scores of fatigue, nausea, vomiting and pain in the observation group were significantly lower than those in the control group(P〈0.05). Conclusion ω-3 polyunsaturated fatty acid is effective in the treatment of colon cancer after radical resection,and it can significantly improve the nutritional level and quality of life of, and has a high value and significance of clinical promotion.
