摘要
目的:探讨HE4和CA125水平变化在卵巢癌术后疗效评估中的应用价值。方法:选取2013年12月至2015年6月我院收治的80例卵巢疾病患者作为研究对象,根据诊断结果分为卵巢癌组(45例)及卵巢良性疾病组(35例),同时选取36例同期门诊女性健康体检者作为对照组,采用电化学发光法和双抗体夹心ELISA法分别测定CA125和HE4水平,比较各组及不同时间点CA125和HE4水平变化。结果:卵巢良性疾病组术前及卵巢癌组血清CA125和HE4水平明显高于对照组,卵巢癌组术前及术后3个月CA125和HE4水平明显高于卵巢良性疾病组,差异有统计学意义(P〈0.05);卵巢癌组Ⅲ~Ⅳ期术前及术后3个月CA125和HE4水平明显高于Ⅰ~Ⅱ期,差异有统计学意义(P〈0.05);卵巢癌组术后病情稳定和缓解患者术后各时间点CA125、HE4水平均明显低于术前1d,术后CA125、HE4水平随时间的延长呈逐渐降低趋势,差异有统计学意义(P〈0.05);卵巢癌组术后病情进展患者术后1周、1个月及3个月CA125、HE4水平均明显低于术前1d,术后CA125、HE4水平随时间的延长呈先降低后升高趋势,差异有统计学意义(P〈0.05)。结论:卵巢癌术后连续监测HE4和CA125水平变化可为疗效评价和病情进展预测提供参考依据,在临床上具有重要应用价值。
Objectives: To investigate the value of HE4 and CA125 level changes in the evaluation of curative effect after operation of ovarian cancer. Method: The 80 ovarian patients in our hospital from December 2013 to June 2015 were selected as research objects,which were divided into ovarian cancer group( 45 cases) and ovarian benign disease group( 35 cases) according to the diagnosis results. At the same time,36 healthy women in our hospital for physical examination were selected as the control group. The levels of CA125 and HE4 were determined by electrochemical luminescence method and double antibody sandwich ELISA method. The changes of CA125 and HE4 levels in different groups and different time points were compared. Results: The serum levels of CA125 and HE4 levels in ovarian benign disease group before surgery and ovarian cancer group was significantly higher than the control group; the levels of CA125 and HE4 in ovarian cancer group before and at 3 months after operation were significantly higher than ovarian benign disease group,both with statistically significant difference( P〈0. 05). The level of CA125 and HE4 in ovarian cancer group at III- IV stage was significantly higher than that at I- II stage before and at 3 months after operation,with statistically significant difference( P〈0. 05). The level of CA125 and HE4 in ovarian cancer group patients who had stable disease after operation at each time point was significantly lower than preoperative 1d; the level of HE4 and CA125 were decreased gradually with the time,both with statistically significant difference( P〈0. 05). The levels of CA125 and HE4 in ovarian cancer group 1 week,1 month and 3months after operation were significantly lower than preoperative 1d with disease progression,and the level of HE4 and CA125 were decreased at first and then increased after surgery,both with statistically significant difference( P〈0. 05). Conclusion: Continuous monitoring of HE4 and CA125 levels after treatment of ovarian cancer can provides reference basis for the prediction of the effect evaluation and the progress of the disease,which has important application value in clinic.
