一次性直视人流手术系统辅助下人工流产术对患者心理、内分泌及疼痛的影响

Impact of one- time observable system assisted abortion on the psychology,endocrine and pain of patients

目的:探讨一次性直视人流手术系统辅助下人工流产术对患者心理、内分泌及疼痛的影响。方法:选取2015年2月至2015年11月在我院行人工流产手术的患者140例,分别采用传统人流手术(对照组)和一次性直视人流手术系统辅助下人工流产术(观察组),每组70例。术前及术后采用视觉模拟评分法(VAS)评价患者疼痛程度,采用焦虑自评量表和抑郁自评量表评价患者的焦虑抑郁情况,并测定患者皮质醇和促肾上腺皮质激素水平,进行统计学分析。结果:术前两组VAS评分、SDS和SAS评分均无明显差异,术后1d观察组VAS评分、SDS和SAS评分均低于对照组,其差异有统计学意义(P<0.05)。术前两组Cor、促肾上腺皮质激素水平无明显差异(P>0.05),术后两组Cor、促肾上腺皮质激素水平均较术前降低,其组内差异有统计学意义(P<0.05);术后观察组Cor、促肾上腺皮质激素水平低于对照组,组间差异有统计学意义(P<0.05)。结论:一次性直视人流手术系统辅助下人工流产术可明显改善术后患者的焦虑和抑郁情绪,降低应激反应程度,同时降低手术创伤,术后恢复快,值得临床推广。

Objectives： To investigate the impact of one- time observable system assisted abortion on the psychology,endocrine and pain of patients. Methods： 140 patients in our hospital from February 2015 to November2015 were selected and divided into traditional abortion（ control group） and one- time observable system assisted abortion group（ study group）,each of 70 cases. Preoperative and postoperative pain was evaluated by visual analog score（ VAS）,and the anxiety and depression level was evaluated with respective self- rating scale,so as the cortisol and adrenocorticotropic hormone levels for statistics analysis. Results： Preoperative VAS score,SDS and SAS scores were not significantly different between the two groups,and the VAS scores,SDS and SAS scores of observation group were lower than the control group,with statistically significant difference（ P〈0. 05）. The Cor,ACTH levels had no significant difference between the two groups before treatment,but these were reduced after operation,with statistically significant difference within each group（ P〈0. 05）,especially in the observation group,with statistically significant difference between the two groups（ P〈0. 05）. Conclusion： one- time observable system assisted abortion can significantly improve postoperative anxiety and depression,lower stress level and reduce surgical trauma. With faster postoperative recovery,it is worthy of promotion.
