脑卒中患者失禁相关性皮炎前瞻性系统护理干预研究

Prophylactic effect of prospective systemic nursing in patients with incontinence-associated dermatitis after stroke

目的探讨前瞻性系统护理在预防脑卒中患者失禁相关性皮炎中的应用效果。方法将2013年12月—2014年12月收治的脑卒中患者32例设为对照组,给予常规护理。将2015年1月—2016年1月收治的脑卒中患者32例设为观察组,给予前瞻性系统护理干预。对比2组失禁相关性皮炎发生率、严重程度、护理后生活质量评分和护理满意度评分。结果观察组失禁相关性皮炎发生率为6.25%(2/32),低于对照组的28.13%(9/32),差异有统计学意义(P<0.05);失禁相关性皮炎严重程度观察组轻于对照组,差异有统计学意义(P<0.05);实施护理干预后观察组简明健康调查表各维度评分高于对照组,差异有统计学意义(P<0.05);观察组护理满意度各项目评分优于对照组,差异有统计学意义(P<0.01)。结论前瞻性系统护理能降低脑卒中患者失禁相关性皮炎发生率,改善生活质量。

Objective To investigate the prophylactic effect of prospective systemic nursing in patients with incontinence-associated dermatitis after stroke. Methods Thirty- two stroke patients with hospital admission from December 2013 to December 2014 were selected as the control group and received routine nursing care. Another 32 stroke patients with hospital admission from January 2015 to January 2016 were selected as the observation group,giving prospective systematic care interventions.The incidence and severity of incontinence-related dermatitis,quality of life scores and nursing satisfaction scores of two groups were observed and compared. Results The incidence of incontinence-associated dermatitis in observation group was lower than that in control group（6. 25% vs. 28. 13%,P〈0.05）. The severity of incontinence-related dermatitis in observation group was lighter than that in control group（ P〈0.05）. The scores of each dimension of 36-item shot-form health status survey in the observation group were higher than those of the control group（ P〈0.05 or P〈0.01）. The score of each dimension in assessment of nursing satisfaction was higher in the observation group than those of the control group（ P〈0.01）. Conclusion Prospective system nursing is effective to reduce the incidence of incontinence-associated dermatitis and to improve the quality of life of patients after stroke.
