限制性输液治疗妇科急性中重度出血的临床观察

Clinical observation of limited infusion in the treatment of gynecological acute moderate and severe hemorrhage

目的：探讨观察限制性输液治疗妇科急性中重度出血的临床效果。方法：从我院2014年5月-2016年5月救治的妇科急性中重度出血患者中抽样选取78例作为研究对象,随机将其等分为试验组与对照组,对照组采用常规补液法,试验组采用限制性输液,观察比较两组的临床效果。结果：治疗后,两组血浆TNF-α、IL-2浓度均明显升高,但对照组升高趋势更为明显,差异有统计学意义（P〈0.05）。试验组患者的输液量、平均动脉压及凝血酶原时间均明显优于对照组,差异有统计学意义（P〈0.05）。试验组患者的满意度、生存率均明显优于对照组,差异有统计学意义（P〈0.05）。结论：对妇科急性中重度出血患者及时予以限制性输液治疗,能有效改善重要脏器血流灌注,大大提高临床治疗效果,减少并发症的发生,有利于患者预后。

Objective： To investigate the clinical effect of limited infusion in the treatment of gynecological acute and severe bleeding. Methods： Selected 78 patients with gynecological acute moderate and severe bleeding in our hospital from May 2014 to May 2016 and randomly divided them into experimental group and control group. The control group was treated with routine rehydration method,while the experimental group was treated with limited infusion. The clinical effect of the two groups was observed and compared. Results： After treatment,the levels of TNF-α and IL-2 density in the plasma of the two groups increased significantly and the increase in the control group was significantly higher than that of the experimental group. The difference was statistically significant（ P〈0.05）. The infusion volume,mean arterial pressure and prothrombin time in the experimental group were significantly higher than that of the control group（ P〈0.05）. The patients＇ satisfaction and survival rate in the experimental group were significantly better than those of the control group（ P〈0.05）. Conclusion： Limited infusion therapy can improve the blood perfusion of vital organs in patients with gynecological acute moderate and severe hemorrhage. It can greatly improve the clinical therapeutic effect and reduce the incidence of complications of the patients; therefore it is beneficial to the prognosis of patients.
