脑卒中后肩-手综合征的中医临床特征及危险因素

Clinical characteristics and risk factors of shoulder hand syndrome in patients with stroke

目的:探讨脑卒中后肩-手综合征患者中医临床特征及影响因素。方法:100例脑卒中后肩-手综合征患者作为研究组,另选取同期诊治脑卒中后未发生肩-手综合征患者50例作为对照组,分析脑卒中后肩-手综合征患者中医临床特征及危险因素。结果:脑卒中后肩-手综合征患者主要中医临床表现有肩痛、手肿胀、上肢活动障碍、手痛、肩手痛、上肢皮温升高、手指活动障碍。单因素分析得出,研究组患者年龄、脑梗死病灶范围大于对照组,肩关节半脱位发生率高于对照组,差异有统计学意义(P<0.05)。多因素分析得出,年龄、脑梗死病变范围、肩关节半脱位均为影响脑梗死后肩-手综合征发生危险因素。结论:脑卒中后肩-手综合征患者以上肢疼痛、活动障碍为临床表现,诊断较易,高龄、脑梗死病变范围大及合并有肩关节半脱为影响脑卒中后肩-手综合征发生危险因素。

Objective： To investigate the clinical characteristics and influencing factors of patients＋with shoulder hand syndrome after stroke. Methods： 100 cases of shoulder hand syndrome after stroke patients were selected as the study group, the diagnosis and treatment of post - stroke patients with shoulder hand syndrome occurred in 50 cases as control group, analysis of shoulder hand syndrome after stroke clinical characteristics and risk factors of patients. Results： Post stroke shoulder hand syndrome were the main TCM clinical manifestations of shoulder pain, hand swelling, upper limb movement disorders, hand pain, shoulder pain, upper limb skin temperature, hand finger movement disorder. Single factor analysis ＂showed that patients in the stud- y group, age, cerebral infarction lesions range greater than the control group, the incidence of shoulder subluxation was higher than the control group, the difference was statistically significant （P 〈 0. 05 ）. Multi factor analysis showed that age, cerebral infarction lesions, shoulder subluxation were af- fected after cerebral infarction of shoulder hand syndrome risk factors. Conclusion： Shoulder hand syndrome after stroke in patients with upper limb pain, movement disorder in clinical manifestations, the diagnosis is easy, age, cerebral infarction range and associated with the shoulder joint half off for the effect of shoulder hand syndrome after stroke risk factors.
