摘要 目的分析胃癌组织中干细胞标志物性别决定相关基因簇2(Sox2)以及外周血树突细胞(DC)功能对预后的影响,为胃癌患者预后的早期评估提供参考依据。方法选取70例胃癌患者及50名健康受试者,使用实时荧光定量多重聚合酶链反应(PCR)检测胃癌组织、正常胃黏膜Sox2表达,并对其外周血DC进行检测。比较两组受试者及不同病理指标胃癌患者间上述指标间差异,应用受试者工作特征曲线(ROC)分析Sox2及DC对患者预后的诊断价值。应用t检验和ROC曲线分析进行统计分析。结果胃癌组织Sox2 mRNA相对表达量显著低于正常胃黏膜组织,差异有统计学意义(P=0.002);患者组外周血DC/PBMC显著低于正常组,差异有统计学意义(P=0.013)。不同TNM分期、细胞分化程度、肿瘤浸润深度、淋巴结转移状态胃癌患者肿瘤组织Sox2、DC计数、DC/PBMC表达比较,差异有统计学意义(P=0.006,0.011,0.007,0.000)。Sox2 mRNA表达和DC计数降低预测患者3年病死的AUC分别为0.877、0.935,P=0.000,0.000,均可有效预测患者预后。结论胃癌组织中Sox2表达、DC计数降低是胃癌患者预后不良的预测因子。 Objective To analyze the effect of gastric cancer stem cell marker sex determining region Y-box 2 (Sox2) and peripheral blood mononuclear dendritic cell (DC) function on the prognosis of gastric cancer, So as to provide a reference for the early evaluation of the prognosis of patients with gastric cancer. Methods 70 patients with gastric cancer and 50 healthy subjects were selected. Real-time fluorescence quantitative polymerase chain reaction (PCR) was used to detect the expression of Sox2 in gastric cancer tissues and normal gastric mucosa. The peripheral blood DC function was examined. The difference between the two groups with different pathological staging was compared. The receiver operating characteristic curve (ROC) was used to analyze the diagnostic value of Sox2 and DC for the prognosis of gastric cancer. Results Relative expression of Sox2 mRNA in gastric cancer was significantly lower than that in normal gastric mucosa (P = 0.002). Peripheral blood DC/PBMC in patient group was significantly lower than that of the normal group (P = 0.013). The difference of Sox2 expression, DC count and DC/PBMC among gastric cancer patients from different TNM stages (P = 0.006), cell differentiation (P = 0.011), tumor invasion depth (P = 0.007) and lymph node metastasis status (P = 0.000) was statistically significant.The area under ROC curve (AUC) when using Sox2 mRNA expression and the decrease of DC count to predict patients'3-year mortality was respectively 0.877 and 0.935 (P 〈 0.05), which can forecast the prognosis of gastric cancer. Conclusion The expression of Sox2 and the decrease of DC count in gastric cancer tissue are prognostic factors of poor prognosis in patients with gastric cancer.
