胃癌术后患者营养状况及生存期对生存质量的影响 被引量:18

摘要 目的比较不同营养状况和生存期的胃癌术后患者的生存质量。方法以96例胃癌患者为研究对象,以生存期1年为界限分组,比较两组患者营养状况和生存质量;以营养状况分组,比较各组生存质量情况,以主观综合性营养评估(SGA)评价患者营养状况;以胃癌患者生存质量问卷(QLQ-STO22)评价患者生存质量。结果生存期﹤1年组术后营养不良的发生率(40.00%)高于生存期≥1年组(17.40%)。QLQ-STO22结果显示生存期﹤1年组呃逆程度(P﹤0.05)和身体外观指数(P﹤0.01)均高于生存期≥1年组。SGA-C组呃逆程度、饮食受限、焦虑和身体外观指数均高于SGA-A组(P﹤0.05)。结论胃癌术后患者营养状况可能影响其生存质量,胃癌患者术后应给予积极有效的营养支持。 Objective To compare the quality of life (QoL) of gastric cancer patients with different nutritional status and survival time after surgery. Method 96 patients with gastric cancer were enrolled in the study, and were stratified by survival time as≥1 year and〈1 year group, of which the nutritional status and QoL were compared;And then all patients were stratified by nutritional status to compare the QoL of each group, of which the nutritional status was evaluated by a score of patient generated-subjective global assessment (SGA), and the QoL by QLQ-STO22. Result The incidence of malnutrition in〈1 year group (40.00%) was higher than that of the≥1 year group (17.40%). QLQ-STO22 showed that score of hiccup (P〈0.05) and body image index (P〈0.01) in〈1 year group were higher than those of the≥1 year group. The score of hiccup (P〈0.01), eating restriction (P〈0.01), anxiety (P〈0.05) and body image index (P〈0.01) in SGA-C group were higher than those of the SGA-A group. Conclusion The nutritional status of patients underwent gastrectomy are associated with their quality of life, and effective nutritional interventions are necessary for gastric cancer patients af-ter surgery.
