急性颅脑损伤患者心电图、血清淀粉酶、乳酸、NT-proBNP、hs-CRP、hs-cTnT的变化和临床意义

Changes and clinical significance of electrocardiogram,serum amylase,lactic acid,NT-proBNP,hs-CRP,hs-cTnT in patients with acute craniocerebral injury

目的研究急性颅脑损伤患者心电图、血清淀粉酶、乳酸、N末端脑钠肽前体(NT-proBNP)、超敏C-反应蛋白(hsCRP)、超敏心肌肌钙蛋白T(hs-cTnT)的变化并探讨其临床意义。方法选取2014年1月至2016年1月收治的急性颅脑损伤患者102例,所有患者均于入院即刻及入院后1、3 d行24 h动态心电图检测,并给予各项血生化检查。结果入院即刻患者心电图、NT-proBNP异常率显著高于入院后1、3 d,且入院后1 d又显著高于入院后3 d;入院即刻患者淀粉酶、乳酸hs-CRP、hs-cTnT水平均显著高于入院后1、3 d,且入院后1 d又显著高于入院后3 d,差异均有统计学意义(P均<0.05)。结论急性颅脑损伤患者可能存在不同程度的心肌损伤,对患者血清淀粉酶、乳酸、NT-proBNP、hs-CRP的变化进行检测,可有助于了解病情、观察疗效以及预后判定,并有利于临床医生制定具有针对性的干预措施,减少患者心肌损伤。

Objective To study the changes of electrocardiogram,serum amylase,lactic acid,NT-proBNP,hs-CRP and hs-cTnT in patients with acute craniocerebral injury,and to investigate its clinical significance. Methods From January 2014 to January 2016,102 patients with acute craniocerebral injury were selected as the observation group. All patients underwent 24 h dynamic electrocardiogram detection at adnitssion immediately,and 1,3 d after admission,and the blood biochemical examination was made. Results The abnormal rates of ECG and NT-proBNP in patients at admission immediately were significantly higher than those on 1,3 d after admission,and those of 1 d after admission were significantly higher than those of 3 d after admission;The levels of amylase,lactic acid hs-CRP,hs-cTnT in patients at admission immediately were significantly higher than those of 1,3 d after admission,and those of 1 d after admission were significantly higher than those of 3 d after admission. The differences were significant（P all 〈 0. 05）. Conclusion There may be different degrees of myocardial injury in patients with acute craniocerebral injury,it the detection of the changes of amylase,lactic acid,NT proBNP,and hs-CRP,can help to understand the disease condition,efficacy and prognosis,and it is conducive to the development of targeted interventions to reduce the myocardial injury.
