电针+拔罐+药物敷贴治疗痛痹583例临床观察

The Clinical Observation of Electroacupuncture Combined with Cupping Glass and Medicinal Compress Paste in the Treatment of Arthromyodynia on 583 Cases

[目的]观察电针＋拔罐＋药物敷贴治疗痛痹疗效。[方法]使用前瞻性设计方法,对583例门诊患者电针＋拔罐＋药物敷贴治疗,颈部局部常选玉枕、天柱、风池、大椎、阿是穴,远端选合谷、风池、外关、手三里;腰腿痛局部常选肾俞、大肠俞、关元俞、气海俞、三焦俞、上次中下髎、阿是穴等,远端选环跳、委中、承山、太溪等;肩关节痛局部常选肩井、肩髎、肩髃、阿是穴,远端选手三里、合谷、风池、外关等;膝关节痛局部常选内外膝眼、阳陵泉、阴陵泉、足三里、阿是,远端选风市、梁丘、丰隆、昆仑等。毫针1.5寸15度平刺和45度对称针刺,得气后经络治疗仪连续波刺激25~45min。取针后在病变部位与针刺部位竹罐拔罐5~15min,取罐后病痛处用祛风散寒止痛散陈醋调匀适量贴穴位处,4-12h/次。连续治疗10d为1疗程。观测临床症状、不良反应。连续治疗1-3疗程,判定疗效。[结果]颈部治愈86例,好转75例,无效8例,总有效率95.27%（161/169）;腰腿治愈108例,好转96例,无效11例,总有效率94.89%（204/215）;肩关节治愈39例,好转34例,无效5例,总有效率93.59%（73/78）;膝关节治愈61例,好转53例,无效7例,总有效率94.21%（114/121）。[结论]电针＋拔罐＋药物敷贴治疗痛痹,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the effect of electroacupuncture combined with cupping glass and medicinal compress paste in the treatment of arthromyodynia. [Method] Using a prospective design method, electroacupuncture combined with cupping glass and medicinal compress paste cure 583 outpatients, the local neck often choosen Yuzhen, Tianzhu, Fengchi, Dazhui, Ashi acupoint, and the distal selected Hegu, Fengchi, Waiguan, Shousanli; Lumbocrural pain local often choosen Shenshu, Dachangshu, Guanyuanshu, Qihaishu, Sanjiaoshu, Shangcizhongxialiao, Ashi acupoint and so on, the distal selected Huantiao, Weizhong,Chengshan, Taixi, etc; Shoulder pain often choosen Jianjing, Jianliao, Jianyu, Ashi acupoint, and the distal selected Shousanli, Hegu, Fengchi, Waiguan,etc; Knee pain local often choosen Neiwaixiyan, Yanglingquan, Yinlingquan, Zusanli, Ashi acupoint, the distal selected Fengshi, Liangqiu, Fenglong, Kunlun and so forth. Using the 1.5 inch needle adopted 15 degrees which was named horizontal insertion of needle and 45 degrees symmetrical acupuncture, and using the continuous wave of the meridian therapeutic apparatus to stimulate for 25 to 45 minutes after having needling sensation. Cupping bamboo jar in the lesion and the acupuncture points 5 to 15 minutes after taking the needles out, then using the expelling wind and cold and stopping pain powder with mature vinegar to mixing thoroughly,and applying proper amount to paste on the acupoints 4 to 12 hours per times. Continuous treatment for 10 days was 1 course of treatment. Observation of clinical symptoms and adverse reactions. Judge curative effect after continuous treatment for 1 to 3 courses of treatment. [Result] The neck of 86 cases were cured, 75 cases were improved, 8 cases were ineffective, and the total effective rate was 95.27 （161/169）. The lumbocrural of 108 cases were cured, 96 cases were improved, 11 cases were ineffective, and the total effective rate was 94.89（204/215）. The shoulder pain of 39 cases were cured, 34 cases were improved, 5 cases were ineffective, and the total effective rate was 93.59（73/78）. The Knee pain of 61 cases were cured, 53 cases were improved, 7 cases were ineffective, and the total effective rate was 94.21（114/121）. [Conclusion] The curative effect of electroacupuncture combined with cupping glass and medicinal compress paste in the treatment of arthromyodynia is satisfactory, having no serious adverse reactions, it is worth promoting.
