Application of ability-quality iceberg model on nursing safety management in pediatric department

目的：初步探讨能力素质冰山模型在儿科护理安全管理中的作用。方法组织科室护理骨干6名成立护理安全管理小组，运用能力素质冰山模型分析2014年1—12月我院儿科所有护理不良事件发生的“人”方面的原因，依据分析结果，采取多种方法提升护士的潜在素质；收集2014年及2015年1—12月反映科室护理安全效果的数据进行统计对比；随机抽查2015年全年护士身份核对执行情况、不规范核对行为，统计护理差错发生率。结果2015年全年护理不良事件发生例数较2014年明显下降（P〈0．05），其中坠床例数下降明显（P〈0．05）。护理纠纷发生例数明显减少（P〈0．05），患者满意度明显上升，身份核对流程执行率为100．00％，不规范核对行为为0．00％，护理差错发生率为0．00％。结论运用能力素质冰山模型分析儿科护理不良事件发生的“人”方面的原因，改进护士培养方法，对护理安全管理有一定成效。

Objective To preliminarily explore the application of ability-quality iceberg model on nursing safety management in the pediatric department .Methods Firstly, a nursing safety management team was established which was comprised of six professional nurses .The ability-quality iceberg model was used to analyze the reasons for all adverse events due to inappropriate nursing emerged in 2014 , in order to improve the quality of nursing care .Moreover, the nursing safety effects were compared between 2014 and 2015 in pediatric department .Lastly, the incidence of nursing mistakes was also an important outcome according to randomly investigating the nurse identity verification and non-standard verification behaviors . Results The nursing adverse events significantly reduced from 2014 to 2015 , especially for cases of falling down from bed （P 〈0.05）.The incidence of nursing disputes also reduced with patient satisfaction increased （P〈0.05）.The nurse identity verification rate was 100.00% and non-standard verification behaviors rate and incidence of nursing mistakes were 0.00%.Conclusions The ability-quality iceberg model can effectively analyze the reasons for adverse events due to inappropriate nursing and can improve the nurse training method as well as the nursing safety management .
