耳鸣患者视觉通道的认知加工

摘要 【目的】探讨耳鸣患者视觉通道的认知加工情况,为日后的耳鸣评估及治疗奠定数据基础。【方法】共入组被试者65名,其中耳鸣组38名(左、右耳鸣组各19名),正常对照组27名。采用点探测任务,并通过操纵任务指令,对两组被试的视觉认知加工进行研究,数据采用SPSS 19.0进行统计分析。【结果】1耳鸣组及对照组在视觉加工的感觉阶段及知觉阶段均未见显著的空间偏向;2在感觉阶段及知觉阶段的反应时均存在显著的组间差异(P=0.01),而转换阶段未见显著的组间差异(P=0.75)。LSD事后检验显示,无论是感觉阶段还是知觉阶段,左侧耳鸣组和右侧耳鸣组的视觉加工速度未见显著差异(P>0.8),而两个耳鸣组均与正常对照组存在显著差异(P≤0.02)。3耳鸣严重程度与视觉加工速度无显著相关。【结论】虽然本研究在行为学数据上未能发现单侧耳鸣患者的视觉加工存在显著的空间偏向,但耳鸣组视觉通道的认知加工与正常对照组存在差异,进一步明确其背后的神经机制有望为耳鸣的评估与治疗带来新的启发。 [ Objective ] To explore the cognitive processing of visual modal in tinnitus patients and then provide data for tinnitus evaluation and treatment in future. [Method] 65 subjects were enrolled in this study, 38 of which composed the tinnitus group, and the other 27 composed the normal control group. Visual cognitive processing was investigated in both groups through dot probe paradigm and appointed task completing. All the acquired behavioral data was analyzed by SPSS 19.0. [Result] There was no significant spatial bias in sensation and perception level of visual processing for both groups. The visual processing was similar in different severity tinnitus patients. The reaction time of tinnitus group was longer than the normal control group in the sensation and perception level of visual processing (P ≤ 0.02), and there was no significant correlation between the severity of tinnitus and visual processing. [ Conclusion ] Although there was significant spatial bias in visual processing in the behavioural data for unilateral tinnitus patients in this study, the visual processing of tinnitus patients group was different from the normal control group, it will be inspirational to probe the potential neuromechanism of these phenomena for tinnitus evaluation and treatment.
