精液过氧化物酶阳性白细胞浓度与精液及精子质量的关系 被引量:5

摘要 【目的】研究男性不育症患者精液过氧化物酶阳性白细胞浓度与精液及精子质量的关系。【方法】回顾性分析近2年来在我院生殖医学中心就诊的男性不育症患者共计164例,根据精液过氧化物酶阳性白细胞浓度的不同将纳入研究的患者分为两组,其中非炎症组(白细胞浓度〈1×10^6/mL)110例,炎症组(白细胞浓度≥1×10^6/mL)54例,两组患者中分别有54例及19例患者因配偶有不明原因的胚胎停育或自然流产史进行了精子DNA碎片率的检测。通过分析和比较两组患者精液及精子主要参数有无差异可以得出相应的结论。【结果】与非炎症组患者相比,炎症组患者精液体积(2.96±1.07,4.16±1.36)、液化时间(22.96±12.55,32.59±16.96)min、精液抗精子抗体(1.82±1.24,4.61±2.53)%、精子DNA碎片异常率(14.81%,47.37%),4项参数均增加,差异有统计学意义(P〈0.05);精子浓度(82.62±47.39,44±33)×10^6/mL、精子活动率(69.96±20.62,46.59±28.58)%、前向运动精子百分率(46.85±19.75,29.58±24.39)%、正常形态精子百分率(2.41±1.33,1.51±1.03)%、精子存活率(74.88±20.79,48.4±27.7)%、精子顶体酶(143.17±51.84,47.49±31.42)Uiu/10^6、精浆锌含量(12.20±6.52,6.89±4.79)mol/次,7项参数均降低,差异有统计学意义(P〈0.05);两组患者的精子总数(235.30±151.65,175.35±143.13)×10^6精液PH(7.43±0.15,7.44±0.12)均无明显变化,差异没有统计学意义(P〉0.05)。【结论】男性不育症患者精液过氧化物酶阳性白细胞浓度升高将导致精液及精子质量下降。 [Objective] To analyze the relationships between the semen peroxidase positive leukocyte concentrations and the semen and sperm qualities of male infertility patients. [Methods] 164 male infertility patients treated in the Center for Reproductive Medicne of our hospital were included in this retrospective clinical study. 110 patients were distributed to the non-inflammatory group, and the left 54 patients assigned to the inflammatory group according to the semen peroxidase positive leukocyte concentrations, and there were respectively 54 and 19 patients with a history of unexplained embryonic damage or spontaneous abortion conducted the sperm DNA fragmentation rates in each group. [ Results ] Compared to the patients of non-inflammatory group, the semen volumes (2.96 ± 1.07, 4.16 ± 1.36)mL, liquefaction time (22.96 ± 12.55, 32.59 ± 16.96) min, positive rates of anti-sperm antibody (1.82± 1.24, 4.61 ± 2.53)% and the abnormal sperm DNA fragmentation rates (14.81%, 47.37%) of inflammatory group were all increased, the differences were statistically significant (P 〈 0.05); The sperm concentrations (82.62 ± 47.39, 44 ± 33)×10^6/mL, sperm activate rates (69.96 ± 20.62, 46.59 ± 28.58)%, progressive sperm percentages (46.85 ± 19.75, 29.58 ± 24.39)%, normal sperm morphology percentages (2.41 ± 1.33, 1.51 ± 1.03)%, sperm survival rates (74.88 ± 20.79, 48.4 ± 27.7)%, sperm acrosome enzyme contents (143.17± 51.84, 47.49 ± 31.42) Uiu/106 and seminal plasma zinc contents (12.20 ± 6.52, 6.89 ± 4.79) mol/ sample of inflammatory group were all reduced, and the differences were statistically significant (P 〈 0.05); There were no statistically significant difference between the total number of sperm (235.30 ± 151.65, 175.35 ± 143.13) ×10^6 and the semen PH (7.43 ± 0.15, 7.44 ±0.12) of the two groups (P 〉 0.05). [Conclusions] The increasing of semen peroxidase positive leukocyte concentrations of male infertility patients would resuh in semen and sperm qualities declined.
