120例重症医学科病房急性肾功能衰竭的临床分析

The Clinical Analysis of 120 Cases of Severe Medical Wards of Acute Renal Failure

目的：分析重症医学科病房（ICU）不同年龄段急性肾功能衰竭（ARF）患者的疾病类型与治疗效果。方法：选择笔者所在医院在2013年12月～2015年12月ICU收治的ARF患者120例，根据年龄将其分为两组，即中青年组（18～59岁）58例与老年组（60及60岁以上）62例。观察对比两组患者不同ARF类型的发生情况及临床疗效。结果：老年组肾前性ARF的发生率为51．61％，高于中青年组的25．86％，差异有统计学意义（P〈0．05）；肾实质性ARF的发生率为29．03％，低于中青年组的51．72％，差异有统计学意义（P〈0．05）。中青年组的治疗总有效率为98．28％，高于老年组87．10％，差异有统计学意义（P〈0．05）。结论：相较于中青年ARF患者，老年ARF患者受脏器功能的影响病情更为严重，且治疗效果不够理想，临床应积极找到病因，尽早进行诊断与治疗，以便改善预后质量。

Objective： To analyze the intensive medicine unit （ICU） of different ages with acute renal failure （ARF） type of disease and patient outcomes. Method： 120 cases of ARF patient were selected from December 2013 to December 2015 admitted to the authors hospital ICU, which according to age were divided into two groups, namely young group（18 to 59 years） in 58 eases, the elderly（60 and 60 years old above） 62 cases.Different types of ARF incidence and clinical efficacy of two groups were observed and compared.Result： The incidence of pre-renal ARF was 51.61%, which was higher than the young group of 25.86%, the difference was statistically significant（P〈0.05）, the incidence of renal parenchymal ARF elderly group was 29.03%, which was lower than the young group of 51.72%, the difference was statistically signifieant（P〈0.05）.Youth treatment group total effective rate was 98.28%, which was higher than the older group of 87.10%, the difference was statistically significant （P〈0.05）.Conclusion： Compared to young patients with ARF, ARF elderly patients suffer organ function in more severe disease, and the treatment effect is not ideal, clinical should actively find the cause, diagnosis and treatment as soon as possible, in order to improve the prognosis quality.
