摘要
目的探讨新疆巴楚县维吾尔族妇女宫颈病变检出率和HPV感染率与当地饮用水中微量元素水平的相关性,为新疆维吾尔族宫颈癌的干预提供依据。方法 2014年3月1日-6月15日,通过整群抽样的方法,在巴楚县的9个乡镇进行维吾尔族妇女宫颈癌筛查,共筛查5 045名妇女。同一年内的2、5、8和12月定点采集每个乡镇的饮用水10份,每份至少1 L。测定饮用水中微量元素Ni、Cu、Zn、Se和Fe的水平,饮用水中微量元素水平与维吾尔族妇女宫颈病变检出率和HPV感染率的相关性采用偏相关分析。结果巴楚县饮用水中Ni、Cu、Zn、Se和Fe微量元素的水平未超过国家生活饮用水的标准。妇女HPV感染的阳性率为9.3%(501/5 045),宫颈病变(CIN2+级)的检出率为1.33%(67/5 045)。CIN2+级宫颈病变的检出率与饮用水中Zn水平呈负相关(rs=-0.597,P=0.016);HPV感染率与饮用水中Cu水平呈正相关(rs=0.528,P=0.034),与Se水平呈负相关(rs=-0.586,P=0.005)。结论巴楚县饮用水中微量元素的水平与当地维吾尔族妇女HPV感染率、高级别宫颈病变的发生具有一定相关性。
Objective To study the relationship between trace elements in drinking water and the incidence of HPV infection and high cervical lesions including cervical cancer in order to provide useful information for intervention of cervical cancer.Methods The samples of drinking water were collected from 9 towns including Bachu,Awati,Selibuya,Qiongkuqiake,Alageer,Akesake,Qiaerbage,Anarekuli and Duo-laitibage in Bachu County of Xinj iang.The concentrations of trace elements of Ni,Cu,Zn,Se and Fe were determined by inductively coupled plasma atomic emission spectroscopy.Correlation between the concen-trations of trace elements in drinking water and the incidence of HPV infection and high cervical lesions in-cluding cervical cancer was performed with partial correlation analysis.Results The concentrations of trace elements (Ni,Cu,Se,Fe and Zn)were in normal scope in Bachu.The concentration of Zn was nega-tively correlated with the incidence of high cervical lesions in drinking water;the concentration of Cu was positively with the rate of HPV infection in drinking water,and Se was negatively correlated with the rate of HPV infection (all P 〈0.05).Conclusion The trace elements in drinking water were correlated with the incidence of HPV infection and the development of cervical lesions in Bachu of Xinjiang.
