摘要
心房颤动(简称房颤)是临床上最常见的心律失常之一,有较高的致死率和致残率。经过近20年发展,导管消融已成为房颤根治的主要手段。肺静脉是房颤发生的最重要触发灶,肺静脉隔离已被广泛接受并被视为房颤消融的基石,实现长久肺静脉电隔离可提高单次手术消融疗效。本文旨在重温肺静脉在房颤中的作用机制,强调肺静脉电隔离,及应用新技术实现持久电隔离,提高房颤单次消融成功率。
Atrial fibrillation(AF)is one of the most common arrhythmia with higher death rate and disability rate in clinic.After 20 years development,Catheter ablation has become an important treatment for atrial fibrillation.Pulmonary veins(PVs)has been the most important source of triggers.And PV isolation(PVI)has been widely accepted and regarded as the cornerstone of the ablation.Complete isolation of PVs can improve the success of AF ablation.The purpose of this paper is to review the mechanism of pulmonary vein in atrial fibrillation,emphasize PVI,and show new technologies to achieve durable PVI to improve the success rate of single ablation.
