摘要
目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者外周血T细胞CD69表达率的检测及意义。方法选择81例经多道睡眠图监测确诊的OSAHS患者,根据呼吸暂停低通气指数(AHI)分为轻度、中度、重度3组,每组27例;27例无OSAHS的打鼾者作为对照组。流式细胞术检测T细胞CD69表达率,分析T细胞CD69表达率与一般资料多道睡眠图监测指标的相关性。结果 1OSAHS患者外周血T细胞CD69表达率随着OSAHS严重程度上升逐渐增加(F=14.736,P<0.01);两两比较,对照组与轻度组间T细胞CD69表达率无明显差别(t=1.649,P>0.05);其余组间比较,均存在明显差别(P<0.05)。2OSAHS患者外周血T细胞CD69表达率和体质量指数、年龄、性别均无相关性(P>0.05),和AHI呈正相关,与最低动脉血氧饱和度呈负相关(P<0.05)。3手术及持续气道正压通气联合治疗的27例重度OSAHS患者AHI及外周血T细胞CD69表达率明显降低,最低动脉血氧饱和度明显上升,存在显著差别(P<0.05)。结论外周血T细胞CD69表达率上升可能为OSAHS并发心血管疾病机制之一,T细胞CD69表达率的检测对OSAHS患者病情程度的反映及并发心血管损害趋势的评估具有一定的临床意义。
OBJECTIVE To investigate the CD69 expression of T cells in peripheral blood and its significance in patients with obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS According to AHI,81 OSAHS patients diagnosed by polysomnography(PSG)were divided into mild,moderate,and severe groups,with 27 cases in each group.Another 27 snoring patients without OSAHS were considered as control group.Flow cytometry was used to detect the CD69 expression rate of T cells.The correlation between the CD69 expression rate of T cells and the PSG indices were analyzed.RESULTS 1.The CD69 expression rate of T cells in peripheral blood of the patients increased gradually with the increased degree of OSAHS(F=14.736,P〈0.01).There was no significant difference in CD69 expression rate of the T cells between the control group and the mild group(t=1.649,P〉0.05).There were significant differences among the other groups(P〈0.05).2.The CD69 expression rate of T cells in peripheral blood of OSAHS patients had no correlation with BMI,age and gender(P〉0.05),but had positively correlation with AHI,negatively correlation with LSaO_2(P〈0.05).3.The CD69 expression rate of T cells and AHI in severe OSAHS patients after a comprehensive treatment decreased significantly,but the LSaO_2increased significantly(P 〈0.05).CONCLUSION Increased CD69 expression in peripheral blood T cells may be one of the mechanisms of OSAHS complicated with cardiovascular disease.The CD69 expression rate of T cells reflects the degree of OSAHS and has some clinical significance in assessment of the risk of cardiovascular damage.
