常规超声、CE及ARFI对乳腺肿瘤诊断价值的对比研究

摘要 目的:研究分析常规超声、压迫式弹性成像(compression elastography,CE)及声脉冲辐射力成像(acoustic radiation force impulse,ARFI)对良恶性乳腺肿瘤的诊断价值。方法 :收集2013年5月至2015年9月医院外科收治的乳腺肿瘤患者78例,肿块共96个,所有肿块均经过病理检查。分别使用常规超声、CE和ARFI对肿瘤进行扫查,随后收集其声像图,计算每种检查方法对乳腺肿瘤诊断的敏感性、特异性、准确性、阳性预测值及阴性预测值。结果:常规超声正确诊断良性肿瘤50个、恶性肿瘤24个,不能诊断的有9个。CE正确诊断良性肿瘤50个、恶性肿瘤33个。ARFI正确诊断良性肿瘤57个、恶性肿瘤34个。ARFI的特异性、敏感性、阳性预测值、阴性预测值最高,相比之下有统计学意义。结论:在常规超声、CE及ARFI 3种方法中,ARFI检查效能最高,值得临床推广应用。 Objective To study the values of conventional ultrasound, compression elastography (CE) and acoustic radiation force impulse (ARFI) for the diagnoses of benign and malignant breast tumors. Methods Totally 78 breast tumor patients from May 2013 to September 2015 were enrolled into the study, and all the 96 tumors involved underwent pathological examination and uhrasonography by conventional ultrasound, CE and ARFI respectively. Then the sonograms were obtained to determine the sensitivities, specificities, accuracies, positive and negative predictive values of conventional ultrasound, CE and ARFI. Results The numbers of benign tumors were 50 found by conventional ultrasound, 50 by CE and 57 by ARFI, and those of malignant tumors were 24 by conventional ultrasound, 33 by CE and 34 by ARFI. Conventional ultrasound also found 9 tumors undefined. ARFI behaved significandy the best in the specificity, sensitivity, positive and negative predictive values. Conclusion ARFI gains advantages over conventional ultrasound and CE, and thus is worthy promoting clinically.
