早产妇不同孕龄对新生儿心肌功能影响的临床研究 被引量:1

摘要 目的探讨早产妇不同孕龄对新生儿心肌功能的影响。方法选取2014年10月至2015年8月于我院出生的孕龄小于36周的早产儿51例为本研究的试验对象,按照孕龄的不同将其分为3组:A组12例(孕龄为24-27周)、B组19例(孕龄为28-31周)和C组20例(孕龄为32-35周)。每例早产儿分别在出生后36-48h、2周及4周时行超声心电图检查,评估其心肌收缩速度(S’)、早期舒张速度(E’)、晚期舒张速度(A’)及短轴缩短率(SF)。结果三组早产儿从出生后48h至4周其S’值、E’值及A’值均有所增加,如B组早产儿S’值由48h3.8±0.9cm/s增加至4周5.0±0.6cm/s。随着A、B和C三组产妇孕龄的增加,早产儿的心肌运动速度呈上升趋势且组间差异显著(P<0.05)。结论新生儿心肌运动功能随早产妇孕龄的增加而越发良好。 Objective To investigate the effect of different gestational ages of women with preterm labor on myocardial function of neonates.Methods 51 stable neonates with less than 36 weeks of gestation born in our hospital from October 2014 to August 2015 were chosen in this study,and according to the difference of gestation were divided into three groups:12neonates in group A(24-27weeks),19 neonates in group B(28-31weeks)and 20 neonates in group C(32-35weeks).Each neonate received the echocardiogram at postnatal 36-48 h,2weeks and 4weeks,respectively.The systolic(S'),early diastolic(E')and late diastolic(A')velocities and shortening fraction(SF)were assessed.Results S'values,E'values and A'values of neonates in three groups from postnatal 48 hto 4weeks all increased,such as S'values in group B increasing from 3.8±0.9cm/s at 48 hto 5.0±0.6cm/s at 4weeks.Along with the increasing of gestation age of these three groups,the myocardial velocity increased with the significant difference between groups(P〈0.05).Conclusion The myocardial function of neonates shows the improvement over the increasing of gestational ages of women with preterm labor.
