血清胱抑素C、视黄醇结合蛋白以及尿微量白蛋白联合检测在糖尿病肾病临床诊断中的意义

Diagnosis significance of cystatin C,retinol binding protein and urinary albumin combined detection for diabetic nephropathy

目的探讨血清胱抑素C、视黄醇结合蛋白以及尿微量白蛋白联合检测在糖尿病肾病患者临床诊断中的应用价值。方法选取2013年6月至2016年5月期间,我院收治的糖尿病患者162例。单纯性2型糖尿病患者85例作为研究组Ⅰ,糖尿病肾病患者77例作为研究组Ⅱ,另选同期在我院进行健康体检的健康人群80例作为对照组。对三组受试者血清胱抑素C、视黄醇结合蛋白和尿微量白蛋白水平进行比较分析,并对糖尿病肾病患者血清胱抑素C、视黄醇结合蛋白和尿微量白蛋白单独检测阳性率及联合检测阳性率进行比较分析。结果与健康对照组相比,研究Ⅰ组Cys C、RBP以及尿mAlb水平均有提高,但差异均不具有统计学意义(P>0.05);而研究Ⅱ组Cys C、RBP以及尿mAlb水平明显提高,差异均具有统计学意义(P<0.05)。与研究Ⅰ组相比,研究Ⅱ组Cys C、RBP以及尿mAlb水平明显提高,差异均具有统计学意义(P<0.05)。同时,77例糖尿病肾病患者中,与血清胱抑素C、视黄醇结合蛋白和尿微量白蛋白联合检测相比,单独检测的阳性率均明显降低,差异均具有统计学意义(P<0.05)。结论血清胱抑素C、视黄醇结合蛋白以及尿微量白蛋白联合检测对糖尿病肾病患者的临床诊断具有重要的指导意义。

Objective To probe into the application value of cystatin C,retinol binding protein and urinary albumin combined detection for diabetic nephropathy.Methods From June 2013 to May 2016,162 diabetes patients were select-ed in our hospital.85 simple type 2 diabetes patients were selected as study groupⅠ.77 diabetic nephropathy patients were selected as study group Ⅱ.During the same period,80 healthy people examination in our hospital were selected as control group.The levels of serum cystatin C,retinol binding protein and urinary albumin of the subj ects in three groups were compared and analyzed.The positive rate of serum cystatin C,retinol binding protein and urinary albumin individu-ally and combined for the diabetic nephropathy patients were compared and analyzed.Results Compared with control group,the levels of serum cystatin C,retinol binding protein and urinary albumin in study groupⅠwere increased,but the differences were not significant statistically（P〉0.05）;the levels of serum cystatin C,retinol binding protein and u-rinary albumin in study groupⅡwere increased significantly,the differences were significant statistically（P〈0.05）. Compared with study group,the levels of serum cystatin C,retinol binding protein and urinary albumin in study groupⅡwere increased significantly,the differences were significant statistically（P〈0.05）.At the same time,in 77 diabetic nephropathy patients,compared with combined detection,the positive rate of serum cystatin C,retinol binding protein and urinary albumin individually was decreased significantly,the differences were significant statistically（P〈0.05）. Conclusion Cystatin C,retinol binding protein and urinary albumin combined detection have important guiding signifi-cance for the clinical diagnosis of patients with diabetic nephropathy.
