非小细胞肺癌与心理社会应激因素的关系

Relationships between Non-small Cell Lung Cancer and Psychosocial Risk Factors

目的探讨生活事件、情绪状态等心理社会应激因素与非小细胞肺癌发病的相关性,为更有效地防治非小细胞肺癌提供科学依据。方法采用病例-对照研究,以经病理确诊为非小细胞肺癌的45例患者为观察组,经病理确诊为非小细胞肺癌的良性疾病45例患者为对照组,采用心理社会应激调查表(PSSG)测试其生活事件、情绪反应;采用焦虑自评量表(SAS)测定其焦虑情况;采用Logistic回归分析非小细胞肺癌与心理社会因素之间的关系。结果多因素Logistic回归分析结果显示,经济困境(OR=3.143)、子女前途问题(OR=7.721)、忧郁(OR=6.122)、绝望(OR=18.215)是非小细胞肺癌的危险因素;而愉快的情绪状态(OR=0.043)是非小细胞肺癌发病的保护性因素。与对照组相比,观察组有严重的焦虑现象,差异有统计学意义(P<0.05)。结论某些负性生活事件、消极情绪是非小细胞肺癌发病的危险因素,而保持心情愉快是非小细胞肺癌发病的保护性因素。

Objective To study the correlations between non-small cell lung cancer and life events, psychosocial stress factors such as emotional state in order to contribute to the prevention and treatment of non-small cell lung cancer. Methods On the basis of case - control study, forty-five pathologically diagnosed cases of non-small cell lung cancer were selected as the ob- servation group while another forty-five cases of non-small cell lung cancer with benign conditions as were in the control group. Psychosocial stress questionnaires were used to collect information on their life events and emotional response. The levels of anxie- ty in each group were evaluated by the self-assessment lists of anxiety （SAS）. Results Muhiariable logistic regression analysis showed that economic plight（ OR = 3. 143 ）, worry about children＇s future （ OR = 7. 721 ）, depression （ OR = 6. 122 ）, and despair （OR = 18. 215 ） were risk factors for non-small cell lung cancer while a pleasant emotional state （OR = 0.043 ） was a protective factor. Compared with the control group, the level of anxiety was quite high among patients of the observation group, and the difference was statistically significant （ P〈0.05 ）. Conclusion Some negative life events and negative emotions are associated with the onset of non-small cell lung cancer. A good mood can reduce the risk of non-small cell lung cancer to some certain extent.
