化浊解毒方对非酒精性脂肪性肝病血清TNF-α炎症因子及肝功能的影响

Effect of Huazhuo Jiedu Decoction on Serum TNF-α,Inflammatory Factors and Liver Function in Patients with Nonalcoholic Fatty Liver Disease

目的:观察化浊解毒方联合常规调脂治疗对非酒精性脂肪性肝病患者血清TNF-α、炎症因子及肝功能的影响。方法:选取我院2011年7月至2015年7月所收治的非酒精性脂肪性肝病患者84例作为研究对象,按照随机数表法分为观察组与对照组,各42例。对照组采取常规调脂治疗(合理运动、低脂饮食、保持规律作息、应用多烯磷脂酰胆碱胶囊)治疗,观察组在对照组基础上联合化浊解毒方,比较两组治疗前后TNF-α、炎症因子IL-6、肝功能指标(AST、ALT)、治疗总有效率、不良反应发生率。结果:两组治疗前TNF-α、IL-6、AST、ALT、中医证候积分的差异均无统计学意义(P>0.05);治疗后两组TNF-α、IL-6、AST、ALT、中医证候积分均降低(P<0.05),同时观察组治疗后的TNF-α、IL-6、AST、ALT、中医证候积分低于对照组(P<0.05)。对照组治疗总有效率为69.05%,低于观察组88.10%,差异有统计学意义(P<0.05)。对照组不良反应发生率为7.14%;观察组为9.52%,差异无统计学意义(P>0.05)。结论:化浊解毒方联合常规调脂治疗可大幅降低非酒精性脂肪性肝病患者的血清TNF-α与IL-6,改善肝功能,临床应用价值大。

Objective： To explore the effect of Huazhuo Jiedu Decoction combined with routine lipid regulation therapy on serum TNF-α,inflammatory factors and liver function in patients with nonalcoholic fatty liver disease.Methods： 84 patients with nonalcoholic fatty liver disease who were treated in our hospital between July 2013 and July 2015 were selected as the study objects.According to the random number table method,the subjects were divided into observation group and controlled group,42 cases ineach group.The controlled group received routine lipid regulation therapy（reasonable exercise,low fat diet,keeping regular work and rest,application of Polyene Phosphatidylcholine Capsules）.On the basis of the controlled group,the observation group was treated with Huazhuo Jiedu Decoction.The TNF-α,inflammatory factor IL-6,liver function indexes（AST,ALT）,the total effective rate of treatment and incidence of adverse reactions were compared between the two groups before and after treatment.Results： There were no significant differences in TNF-α,IL-6,AST,ALT,symptom score between the two groups before treatment（P〉0.05）; After treatment,TNF-α,IL-6,AST,ALT and symptom score in the two groups decreased（P〈0.05）.Meanwhile,TNF-α,IL-6,AST,ALT and symptom score in the observation group after treatment were lower than those in the controlled group（P〈0.05）.The total effective rate of the controlled group（69.05%） was lower than that in the observation group（88.10%）（P〈0.05）.The incidence of adverse reactions in the controlled group was 7.14% and in the observation group was9.52%.The difference was not statistically significant（P〉0.05）.Conclusion： Huazhuo Jiedu Decoction combined with routine lipid regulation therapy can greatly reduce the levels of serum TNF-α and IL-6and improve liver function in patients with nonalcoholic fatty liver disease.The clinical application value is high.
