多发性骨髓瘤致微柱凝胶、凝聚胺法配血困难1例

One case report of the cross matching difficulty in micro-column gel agglutination assay and Polybrene test caused by Multiple myeloma

目的:探讨多发性骨髓瘤致配血困难的解决方法。方法:对此例患者同时进行微柱凝胶法和凝聚胺法的交叉配血,并对凝聚胺法的混悬液用生理盐水进行稀释,以排除异常蛋白的干扰。结果:多发性骨髓瘤患者用微柱凝胶卡配血会出现凝集,用凝聚胺法配血在镜下会有缗钱状聚集,用生理盐水稀释的后缗钱状聚集消失,配血通过。结论:多发性骨髓患者产生的异常蛋白会干扰配血结果,必须用多种方法,去除干扰,确保配血的安全与及时。

Objective：To discuss the solution of cross matching difficulty caused by Multiple myeloma.Methods： Using micro-column gel agglutination assay and Polybrene test to do the cross matching for the patient, and Polybrene suspension was diluted with saline to eliminate interference abnormal protein.Results：Using micro-column gel agglutination assay for the Multiple myeloma patient occurred agglutination, and Polybrene test occurred rouleau formation under microscope, which would disappear after diluting with saline.Conclusions：The abnormal protein produced by Multiple myeloma patient will interfere the results of cross matching, we must use a variety of methods to remove the interference, to ensure cross matching safety and immediate.
