注射用胰岛素在肺结核合并糖尿病中的疗效评价

Evaluation on the curative effect of insulin for injection in the treatment of pulmonary tuberculosis complicated with diabetes

目的 探讨注射用胰岛素在肺结核合并糖尿病治疗中的效果。方法 本研究采用前瞻式随机对照分组研究方法,收集我所在2010年10月-2014年9月4年间完成治疗的肺结核合并糖尿病患者82例的临床资料进行分析,随机分为两组。在正规抗结核治疗同时,实验组予注射用胰岛素,对照组继续以口服降糖药为主,比较两组的治疗费用、中断治疗比例及疗效。结果 实验组患者血糖控制较平稳,6个月涂阴的概率高于对照组,中断治疗、严重肝损害的概率均低于对照组,差异有统计学意义（P〈0.05）。结论 注射用胰岛素能更精确、有效地控制血糖,副作用更小,疗效更好,对保证结核的化疗有重要意义。

Objective To explore and study the curative effect of insulin for injection in the treatment of pulmonary tuberculosis complicated with diabetes. Methods The prospective randomized controlled method was adopted in this study. and clinical data of 82 patients with pulmonary tuberculosis complicated with diabetes who were treated in our institute within 4 years from October 2010 to September 2014 were collected and analyzed. They were divided into two groups. Besides the regular anti-tuberculosis therapy, the experimental group was given insulin for injection while the control group was given oral hypoglycemic drugs as usual. Cost of treatment, incidence of treatment withdrawal and therapeutic effect were compared between the two groups. Results The blood sugar in the experimental group was stably controlled and the 6-month smear negative rate was higher than that in the control group. The incidence of treatment withdrawal and serious liver damage was lower than that in the control group, there were statistically significant differences（P〈0.05）. Conclusion Insulin for injection can accurately and effectively control patients＇ blood glucose, with minor adverse effect and better therapeutic effect, which plays an important role in the chemotherapy of tuberculosis.
