米力农治疗充血性心力衰竭临床效果分析

Clinical effectiveness analysis of Milrinone in treatment of heart failure

目的探讨米力农治疗充血性心力衰竭的疗效和安全性。方法选择98例心力衰竭患者,按治疗时间随机分为观察组和对照组各49例,均在常规治疗基础上观察组加用米力农进行治疗,对照组应用多巴酚丁胺给予治疗,7d后比较治疗结果。结果超声心动图所测心搏量(SV)、心排量(CO)、心指数(CI)等心功能指标明显改善,观察组治疗后显著高于治疗前(P<0.01),且明显高于对照组治疗后(P<0.05)。用Swan-Ganz导管所测血流动力学效应,示心脏指数(CI)和每博容量指数(SVI)明显增加(P<0.05),肺毛细血管楔压(PCWP)、肺动脉压力(PAP)、外周血管阻力(SVR)明显降低(P<0.05),心率和血压无明显变化(P>0.05)。结论米力农治疗心力衰竭,无论是从临床表现还是在心功能改善方面,均产生了良好的治疗效果,且在治疗过程中无明显不良反应发生。

Objective To investigate the efficacy and security of Milrinone in treating congestive heart failure. Methods We had selected 98 cases of patients with heart failure. Accorded to the treatment time were randomly divided into observation group and control group all 49 cases, 2 groups were on the basis of conventional therapy, the observation group were treated with milrinone, the control group should be treated with dobutamine, analyzed the difference after 7 days treatment of the 2 groups. Results Stroke volume （SV） ,cardiac output （CO） ,cardiac index （CI） measured by echocardiography were significantly improved cardiac function. The observation group after treatment was significantly higher than the before treatment（P〈0.01 ）, the observation group after treatment was significantly higher than the control treatment （P〈0.05）. Measured the hemodynamic effects of using Swan-Ganz catheter, a significant increase in stroke volume index of CI and SVI （P〈0.05）, significantly lower of PCMP, PAP and SVR （P〈0.05）, heart rate and blood pressure has no significant changes （P〉0.05）.Conclusions Whether from the clinical manifestations and the heart function improvement, Milrinone treatment of heart failurehave produced a good therapeutic effect, and no significant adverse events during treatment.
