HoLEP治疗老年重度前列腺增生的效果及其安全性分析 被引量:7

摘要 目的观察经尿道前列腺钬激光剜除术(Holmium laser enucleation of prostate,HoLEP)治疗老年重度前列腺增生的效果及其安全性.方法选取110例60岁以上重度前列腺增生(前列腺>80 g)患者,随机分为观察组(行HoLEP治疗)和对照组(行经尿道前列腺汽化电切术即TURP治疗),每组各55例.观察组患者用HoLEP法治疗,对照组患者用TURP法治疗,然后比较两组患者的症状改善以及手术安全性情况.结果两组患者术前的最大尿流率(Qmax)、国际前列腺症状评分(IPSS)、生活质量评分(QOL)、剩余尿量(RU)均无明显差异,术后均较治疗前明显改善,但组间比较无明显差异.结论采用HoLEP术治疗老年重度前列腺增生的效果与TURP相当,手术安全性高,患者更加耐受. Objective To observe the effectiveness and safety of HoLEP( Holmium laser enucleation of prostate) in the treatment of severe benign prostatic hyperplasia( BPH) in the elderly. Method 110 Severe BPH patients who were older than 60 years were selected.They were randomly divided into an observation group and a control group,55 patients in each group. Patients in the observation group were treated with HoLEP and those in the control group were treated with TURP. The outcomes and safety of therapies in the two groups were evaluated by using the subjective / objective improvement of symptoms. Results There was no significant difference in the baseline characteristics between two groups,such as Qmax,IPSS grade,QOL grade,and RU.All the four parameters mentioned above after the surgery were significantly improved during the entire follow-up period in the same group compared with those before the surgery,but there was no significant difference in these parameters between the two groups. Conclusion HoLEP is effective in dealing with severe benign prostatic hyperplasia in the elderly.The subjective functional results appeared the same as those of patients who received the treatment with TURP.Moreover,it appears that HoLEP is safe and well tolerated by patients.
