彩色多普勒在早期糖尿病肾病中的应用

摘要 目的探讨在早期糖尿病肾病中采用超声检查的意义。方法 68例早期糖尿病肾病患者作为观察组,30例健康人作为对照组,超声测定所有患者肾长径、宽径、体积,以及肾动脉、段动脉和叶间动脉收缩期峰值流速(PSV)、舒张期末血流速度(EDV),并计算搏动指数(PI)和阻力指数(RI)。测定观察组血清超敏C反应蛋白(hs-CRP)。结果观察组患者的肾脏长径为(119.2±12.3)mm、宽径为(64.1±9.6)mm、体积为(193.4±17.4)mm^3均大于对照组的(108.3±11.2)mm、(53.2±9.2)mm、(163.8±16.2)mm^3,差异有统计学意义(P<0.05)。观察组肾动脉、段动脉、叶间动脉的PSV和EDV均小于对照组,差异有统计学意义(P<0.05)。观察组肾动脉、段动脉、叶间动脉的RI和PI均高于对照组,差异有统计学意义(P<0.05)。观察组血清hs-CRP为(9.61±1.37)mg/L,与叶间动脉的RI存在正相关关系(r=0.273,P<0.05)。结论早期糖尿病肾病肾血流已出现异常,超声检查可以判断病情严重程度。 Objective To investigate significance of ultrasound examination applied in early diabetic nephropathy. Methods There were 68 patients with early diabetic nephropathy as observation group and 30 healthy people as control group. Ultrasound was applied to detect kidney long diameter, wide diameter, volume, renal artery, segmental artery, interlobar artery, peak systolic velocity (PSV), end diastolic velocity (EDV), pulsatility index (PI) and resistance index (RI). Serum high-sensitivity C-reactive protein (hs-CRP) was detected in the observation group. Results The observation group had kidney long diameter as (119.2 ± 12.3) mm, widediameter as (64.1 ± 9.6) ram, and volume as (193.4 ± 17.4) mm3, which were all larger than (108.3 ± 11.2) mm, (53.2 ± 9.2) mm and (163.8 ± 16.2) mm3 in the control group, and their difference had statistical significance (P〈0.05). The observation group had all lower PSV and EDV of renal artery, segmental artery and interlobar artery than the control group, and the difference had statistical significance (P〈0.05). The observation group had higher RI and PI of renal artery, segmental artery and interlobar artery than the control group, and the difference had statistical significance (P〈0.05). The observation group had serum hs-CRP as (9.61 ± 1.37) mg/L, which was positively correlated with RI of interlohar ai-tery (r=0.273, P〈0.05). Conclusion Due to abnormal renal blood flow in early diabetic nephropathy, ultrasound examination can judge severity of disease.
